癌症病人主要照顾者预期性悲伤影响因素的Meta分析

Influencing factors of anticipatory grief in primary caregivers of cancer patients:a Meta-analysis

目的:通过Meta分析的方法明确癌症病人主要照顾者预期性悲伤的影响因素。方法:使用计算机检索中国知网(CNKI)、中国生物医学文献数据库、万方数据库、维普数据库、the Cochrane Library、PubMed、Web of Science、EMbase,检索时限自建库至2022年2月,关于癌症病人主要照顾者预期性悲伤影响因素的所有文献及纳入文献的参考文献,由2名研究者按文献质量评价标准对提取文献进行质量评价,采用RevMan 5.4进行Meta分析。结果:共纳入21篇文献,累计7119例。Meta分析结果显示,照顾者性别[MD=-4.47,95%CI(-8.81,-0.14)]、与病人的关系[MD=-9.14,95%CI(-12.55,-5.73)]、照顾者年龄[MD=-8.17,95%CI(-11.69,-4.64)]、病人自理能力[MD=-4.58,95%CI(-8.23,-0.92)]、人均月收入[MD=5.81,95%CI(3.99,7.62)]、文化程度[MD=5.80,95%CI(1.38,10.21)]、照顾者性格[MD=3.98,95%CI(0.66,7.30)]是癌症病人主要照顾者预期性悲伤的影响因素。结论:现有证据表明,影响癌症病人主要照顾者预期性悲伤的因素多样,医护人员应早期识别高危人群,对可控影响因素进行有效干预,以减少预期性悲伤的发生。

Objective:To identify the influencing factors of anticipatory grief in primary caregivers of cancer patients by Meta-analysis.Methods:All literature on the factors affecting the anticipatory grief in primary caregivers of cancer patients and the references included in the literature were retrieved from CNKI,CBM,WanFang Database,VIP,the Cochrane Library,PubMed,Web of Science,EMbase.The retrieval time was from the establishment of the database to February 2022.Two researchers evaluated the quality of the extracted literature according to the literature quality evaluation criteria,and performed Meta-analysis using RevMan 5.4.Results:A total of 21 articles were included,with a sample size of 7119 cases.The Meta-analysis results showed that there were 7 influencing factors of the anticipatory grief in primary caregivers of cancer patients:gender of caregiver[MD=-4.47,95%CI(-8.81,-0.14)],relationship with patients[MD=-9.14,95%CI(-12.55,-5.73)],caregiver age[MD=-8.17,95%CI(-11.69,-4.64)],patient self-care ability[MD=-4.58,95%CI(-8.23,-0.92)],per capita monthly income[MD=5.81,95%CI(3.99,7.62)],education[MD=5.80,95%CI(1.38,10.21)],character[MD=3.98,95%CI(0.66,7.30)].Conclusion:Current evidence shows that there are various factors that affect the anticipatory grief of the primary caregivers of cancer patients.Medical staff should identify high-risk groups early and effectively intervene in controllable influencing factors to reduce the occurrence of anticipatory grief.
