乳腺癌患者改良根治术后实施调强放疗与常规放疗的效果比较

Effect comparison of intensity-modulated radiotherapy and conventional radiation therapy after modified radical mastectomy for breast cancer patients

目的比较乳腺癌患者改良根治术后实施调强放疗与常规放疗的效果。方法回顾性抽取2020年1月至2022年5月在国文(长春)国际医院接受乳腺癌改良根治术治疗的46例乳腺癌患者,采用常规放疗的23例患者纳入对照组,采用调强放疗的23例患者纳入观察组,比较两组患者的近期疗效、心脏和患侧肺脏受照剂量、肌钙蛋白水平。结果观察组总有效率为91.30%,高于对照组的78.26%,但差异无统计学意义(P>0.05)。与对照组比较,观察组心脏以及患侧肺脏受照剂量较低(P<0.05)。放疗后,两组肌钙蛋白水平均降低(P<0.05)。与对照组比较,观察组肌钙蛋白水平更低(P<0.05)。结论乳腺癌患者改良根治术后行调强放疗与常规放疗疗效相当,而调强放疗方案并不会对心脏、肺脏产生较大影响,能有效减轻患者心脏、肺脏放射性损伤,值得临床借鉴并推广。

Objective To compare the effect of intensity-modulated radiation therapy and conventional radiotherapy after modified radical mastectomy for breast cancer patients.Methods From January 2020 to May 2022,46 patients with breast cancer who received modified radical mastectomy in Guowen(Changchun)International Hospital were selected retrospectively.23 patients treated with conventional radiotherapy were included in the control group,and 23 patients treated with intensity-modulated radiation therapy were included in the observation group,and the recent efficacy,irradiated dose to the heart and affected lung,and troponin levels of the two groups were compared.Results The total effective rate of the obs ervation group was 91.30%,which was higher than 78.26%of the control gr oup,but the difference was not statistically significant(P>0.05).Compared with the control group,the heart as well as the affected lung was irradiated at a lower dose in the observation group(P<0.05).After radiotherapy,troponin levels decreased in both groups(P<0.05).Troponin level was lower in the observation group compared with the control group(P<0.05).Conclusion The efficacy of intensity-modulated radiation therapy after modified radical mastectomy for breast cancer patients is comparable to that of conventional radiotherapy,and the intensity-modulated radiation therapy regimen does not have a significant impact on the heart and lungs,which can effectively reduce the radiation damage to the heart and lungs of patients,and is worthy of clinical reference and prom otion.
