小剂量阿司匹林联合硫酸镁治疗早发型子痫前期对患者凝血功能改善的作用研究

Effect of low-dose aspirin combined with magnesium sulfate on improving coagulation function in patients with early-onset preeclampsia

目的分析小剂量阿司匹林联合硫酸镁治疗早发型子痫前期(EOPE)的作用。方法选取42例EOPE患者,根据随机数字表法分为A组与B组,每组21例。A组采用阿司匹林(小剂量)联合硫酸镁治疗,B组单用硫酸镁治疗。比较两组临床疗效、凝血功能指标、微炎症状态指标及血压与尿蛋白定量。结果A组总有效率95.24%明显高于B组的71.43%,差异有统计学意义(P<0.05)。治疗前,两组凝血酶原时间(PT)、凝血酶时间(TT)、活化部分凝血活酶时间(APTT)、血小板计数(PLT)、D-二聚体(D-D)与纤维蛋白原(FIB)指标比较,差异无统计学意义(P>0.05);治疗后,A组的PT、TT指标长于B组,APTT指标短于B组,PLT指标、D-D与FIB指标低于B组,差异有统计学意义(P<0.05)。A组的各项微炎症状态指标均低于B组,疗程结束后,A组的舒张压、收缩压水平以及24h尿蛋白定量均低于B组,差异有统计学意义(P<0.05)。结论EOPE患者采用小剂量阿司匹林联合硫酸镁治疗能够改善患者的凝血功能与微炎症状态,具有降压和纠正蛋白尿等作用,具有确切疗效。

Objective To analyze the effect of low dose aspirin combined with magnesium sulfate on early onset preeclampsia(EOPE).Methods 42 patients with EOPE were selected and divided into group A and group B according to a random number table,with 21 patients in each group.Group A was treated with aspirin(small dose)combined with magnesium sulfate,group B was treated with magnesium sulfate alone.The clinical efficacy,coagulation function indexes,microinfammatory status indexes,blood pressure and urinary protein quantification were compared between the two groups.Results The total effective rate of 95.24%in group A was significantly higher than 71.43%in group B,and the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in prothrombin time(PT),thrombin time(TT),activated partial thrombin time(APTT),platelet count(PLT),D-dimer(D-D)and fibrinogen(FIB)indexes between the two groups(P>0.05).After treatment,PT and TT indexes in group A were higher than those in group B,APTT indexes were shorter than those in group B,and PLT indexes,D-D and FIB indexes were lower than those in group B,with statistical significance(P<0.05).The indexes of microinflammatory state in group A were lower than those in group B.After treatment,the levels of diastolic blood pressure,systolic blood pressure and 24 h urinary protein in group A were lower than those in group B,with statistical significance(P<0.05).Conclusion In EOPE patients,low-dose aspirin combined with magnesium sulfate treatment can improve the coagulation function and micro-inflammatory state of patients,and has the effect of reducing blood pressure and correcting albuminuria.
