术后呼吸肌耐力训练对肺癌切除术患者术后通气能力及并发症的影响

Effect of Postoperative Respiratory Muscle Endurance Training on Postoperative Ventilation Ability and Complications in Patients Undergoing Lung Cancer Resection

摘要 目的评估术后呼吸肌耐力训练(respiratory muscle endurance training,RMET)对接受肺癌(lung cancer,LC)切除术患者的呼吸肌能力、术后并发症和住院时间的影响。方法本研究方便选取2022年1—6月福建省南平市第一医院收治的150例LC患者,依据患者术后是否接受RMET,将150例患者分为两组。其中,训练组77例,对照组73例。训练组在1周常规胸部物理治疗的基础上进行为期1周的术后RMET,而对照组仅进行1周常规胸部物理治疗。对比两组患者RMET前后通气能力、术后并发症和住院时间。结果RMET前,两组间分钟通气量(VE)和耐力时间(ET)水平比较,差异无统计学意义(P>0.05)。RMET后,与对照组相比,训练组VE[(80.7±2.6)L/min vs(99.6±3.3)L/min]和ET[(1140.5±144.2)s vs(1367.2±165.2)s]水平均显著增加,差异有统计学意义(t=38.824、8.934,P<0.001)。训练组的住院时间和术后肺部并发症的数量均优于对照组,差异有统计学意义(P<0.05)。结论LC患者术后行RMET可提高呼吸肌耐力,减少术后肺部并发症。 Objective To evaluate the effects of postoperative respiratory muscle endurance training(RMET)on respiratory muscle capacity,postoperative complications and length of stay in hospital in patients undergoing lung cancer(LC)resection.Methods From January to June 2022,150 patients with LC admitted to Nanping First Hospital were conveniently selected in this study.Based on whether patients received RMET after surgery,150 patients were divided into two groups.Among them,77 cases were in the training group and 73 cases were in the control group.The training group underwent 1 week of postoperative RMET on top of 1 week of conventional chest physiotherapy,while the control group underwent only 1 week of conventional chest physiotherapy.The ventilation capacity,postoperative complications and length of hospital stay before and after RMET were compared between the two groups of patients.Results Before RMET,there was no significant difference in minute ventilation(VE)and endurance time(ET)levels between the two groups(P>0.05).After RMET,compared with the control group,VE[(80.7±2.6)L/min vs(99.6±3.3)L/min]and ET[(1140.5±144.2)s vs(1367.2±165.2)s]levels were significantly increased in the training group,and the difference was statistically significant(t=38.824,8.934,P<0.001).The length of hospital stay and the number of postoperative pulmonary complications in the training group were significantly better than those in the control group,and the difference was statistically significant(P<0.05).Conclusion RMET performed in LC patients after surgery can improve respiratory muscle endurance and reduce postoperative pulmonary complications.
