厄贝沙坦氢氯噻嗪联合糖皮质激素治疗重症心力衰竭患者的临床疗效分析

Clinical Analysis of Irbesartan Hydrochlorothiazide Combined with Glucocorticoid in the Treatment of Severe Heart Failure

目的探究厄贝沙坦氢氯噻嗪联合糖皮质激素对重症心力衰竭患者的临床治疗效果。方法便利选取2020年3月—2022年3月江苏省睢宁县中医院收治的79例重症心力衰竭患者作为研究对象,依据随机数表法将其分为对照组39例和观察组40例。两组患者均给予强心、利尿及血管扩张药物等常规基础性药物治疗,对照组增服厄贝沙坦氢氯噻嗪,观察组在对照组基础上口服醋酸泼尼松片,对比两组患者的临床疗效。结果治疗后,观察组LVEDD(46.62±4.73)mm、LVESD(35.47±2.85)mm低于对照组(54.47±5.29)、(42.31±2.11)mm,LVEF(47.18±2.37)%高于对照组(45.23±2.81)%,差异有统计学意义(t=6.957、12.099、3.330,P<0.05)。治疗后,观察组神经内分泌因子水平COR、ALD均优于对照组,治疗有效率高于对照组,差异有统计学意义(P<0.05)。结论厄贝沙坦氢氯噻嗪对治疗重症心力衰竭患者有一定效果,但联合糖皮质激素治疗效果更佳,疗效确切。

Objective To explore the clinical effect of irbesartan hydrochlorothiazide combined with glucocorticoid on patients with severe heart failure.Methods 79 patients with severe heart failure admitted to Suining County Traditional Chinese Medicine Hospital in Jiangsu Province from March 2020 to March 2022 were conveniently selected as the research subjects.They were divided into a control group of 39 cases and an observation group of 40 cases using a random number table method.Both groups of patients were given conventional basic treatment such as cardiotonic,diuretic and vasodilator drugs.The control group was given additional irbesartan and chlorothiazide,and the observation group was given prednisone acetate tablets on the basis of the control group.The clinical efficacy of the two groups of patients was compared.Results After treatment,the LVEDD(46.62±4.73)mm and LVESD(35.47±2.85)mm in the observation group were lower than those in the control group(54.47±5.29)and(42.31±2.11)mm,while LVEF(47.18±2.37)%was higher than that in the control group(45.23±2.81)%,and the difference was statistically significant(t=6.957,12.099,3.330,P<0.05).After treatment,the levels of neuroendocrine factors COR and ALD in the observation group were better than those in the control group,and the treatment effectiveness rate was higher than that in the control group,the difference was statistically significant(P<0.05).Conclusion Irbesartan hydrochlorothiazide has a certain effect in the treatment of patients with severe heart failure,but combined with glucocorticoid therapy has better effect,the efficacy is accurate.
