摘要
目的探讨3D腔镜甲状腺全切术治疗分化型甲状腺癌的疗效。方法将127例分化型甲状腺癌患者按手术方法的不同分为观察组(n=65,3D腔镜甲状腺全切术)和对照组(n=62,2D腔镜甲状腺全切术)。比较两组患者的手术相关指标、甲状腺激素水平以及术后并发症发生情况。结果观察组患者手术时间明显短于对照组,术中出血量明显少于对照组,差异均有统计学意义(P<0.01)。治疗后,两组患者三碘甲状腺原氨酸、甲状腺激素水平均下降,促甲状腺激素水平均上升,差异均有统计学意义(P<0.05)。两组患者并发症总发生率比较,差异无统计学意义(P>0.05)。结论3D腔镜甲状腺全切术可有效缩短分化型甲状腺癌患者的手术时间,减少术中出血量,且其对甲状腺激素的影响及术后并发症均与2D腔镜甲状腺全切术相当,临床可考虑将其推广应用。
Objective To investigate the efficacy of 3D endoscopic total thyroidectomy in the treatment of differentiated thyroid cancer.Method A total of 127 patients with differentiated thyroid cancer were divided into observation group(n=65,3D endoscopic total thyroidectomy)and control group(n=62,2D endoscopic total thyroidectomy)according to different surgical methods.The operation-related indicators,thyroid hormone levels,and postoperative complications were compared between the two groups.Result The operation time in the observation group was significantly shorter than that in the control group,and intraoperative blood loss volume in the observation group was significantly less than that in the control group,and the differences were statistically significant(P<0.01).After the treatment,the levels of triiodothyronine and thyroid hormone in the two groups decreased,while thyroid-stimulating hormone increased,and the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion The 3D endoscopic total thyroidectomy could effectively shorten the operation time and reduce intraoperative blood loss volume,and its effect on thyroid hormones and postoperative complications are comparable to those of 2D endoscopic surgery and is worthy of clinical application.
