血糖波动对缺血性脑卒中进展影响的研究现状

摘要 高血糖水平可能激活机体的氧化应激反应,不利于血管内皮细胞组织的修复,易形成动脉粥样硬化导致脑血管变窄,继而触发脑卒中。然而,近年来有研究指出糖尿病引起的血糖波动也可能加重患者的氧化应激,增加缺血性脑卒中的发生风险。以往临床对缺血性脑卒中相关糖尿病高危因素的控制重点放在抑制患者的高血糖水平上,对血糖波动造成的危害认识不够,可能造成血糖控制不良,导致缺血性脑卒中的发病或预后不良。本文综述了近年来的相关研究,以提升人们对血糖波动的认识,了解其在缺血性脑卒中进展与预后中的作用,指导临床更有效地进行血糖控制。 The high blood sugar level of diabetes may activate the oxidative stress reaction of the patient’s body,which is not conducive to the repair of vascular endothelial cell tissue,and is prone to the formation of athero⁃sclerosis,resulting in narrowing of cerebral vessels,and then triggering stroke.However,in recent years,some studies have pointed out that the fluctuation of blood glucose caused by diabetes may also aggravate the oxidative stress of pa⁃tients and increase the risk of ischemic stroke.In the past,the focus of medical control on the high risk factors of dia⁃betes related to ischemic stroke was to inhibit the high blood sugar level of patients.Insufficient understanding of the harm caused by blood sugar fluctuations may lead to poor blood sugar control,which may lead to the onset or poor prognosis of ischemic stroke.Based on this,this article reviews the relevant research in recent years to improve people’s understanding of blood glucose fluctuations,understand their role in the progress and prognosis of ischemic stroke,and guide clinical more effective blood glucose control.
