前外侧入路与后外侧入路全髋关节置换术的临床效果分析

Clinical Outcome Analysis of Anterolateral Approach and Posterolateral Approach Total Hip Arthroplasty

目的:分析前外侧入路与后外侧入路全髋关节置换术的应用效果。方法:选取2018年1月—2020年12月来宾市兴宾区人民医院收治的行全髋关节置换术患者200例作为研究对象,依据随机数字表法分为对照组和观察组,各100例。对照组行后外侧入路全髋关节置换术治疗,观察组行前外侧入路全髋关节置换术治疗。比较两组术后并发症发生率、手术指标、疼痛评分与髋关节功能。结果:两组术后并发症发生率比较,差异无统计学意义(P>0.05)。观察组切口长度、手术时间和住院时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.001)。观察组患者术后24 h、1周、1个月疼痛评分均低于对照组,差异有统计学意义(P<0.001)。观察组髋关节功能评分高于对照组,差异有统计学意义(P<0.001)。结论:前外侧入路与后外侧入路全髋关节置换术均有较好治疗效果,但前外侧入路全髋关节置换术的治疗效果更显著,能够缩短住院时间和手术时间,减少术中出血量,缩小切口长度,减轻疼痛,促进患者术后尽早恢复,临床应用价值显著。

Objective:To analyze the application effect of anterolateral approach and posterolateral approach for total hip arthroplasty.Methods:Two hundred patients admitted to Xingbin District People's Hospital from January 2018 to December 2020 who underwent total hip arthroplasty were selected as the study subjects and divided into control group and observation group based on the random number table method,with one hundred cases each.The control group was treated with posterolateral approach total hip arthroplasty and the observation group was treated with anterolateral approach total hip arthroplasty.The postoperative complication rate,surgical index,pain score and hip function were compared between the two groups.Results:There was no statistically significant difference in the incidence of postoperative complications between the two groups(P>0.05).The incision length,operation time and hospital stay in the observation group were shorter than those in the control group,and the intraoperative bleeding was less than that in the control group,with statistically significant differences(P<0.001).The pain scores of patients in the observation group were lower than those in the control group at 24 h,1 week and 1 month after surgery,with statistically significant differences(P<0.001).The hip function score in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.001).Conclusion:Both anterolateral approach and posterolateral approach total hip arthroplasty have better therapeutic effects,but anterolateral approach total hip arthroplasty has more significant therapeutic effects,can shorten hospitalization and operation time,reduce intraoperative bleeding,reduce incision length,alleviate pain,and promote patients'early recovery after surgery,and has significant clinical application value.
