电刺激生物反馈及阴道哑铃Kegel锻炼治疗对产后盆底功能障碍性患者盆底肌力、压力性尿失禁的影响

Effects of Electrical Stimulation Biofeedback and Vaginal Dumbbell Kegel Exercise Therapy on Pelvic Floor Muscle Strength and Stress Incontinence in Patients with Postpartum Pelvic Floor Dysfunction

目的探讨电刺激生物反馈+阴道哑铃Kegel锻炼应用于产后盆底功能障碍治疗中的价值。方法方便选取2021年1月—2022年6月漳州市第二医院产后盆底功能障碍患者96例,以随机数字表法将其分成对照组和观察组,各48例。对照组开展阴道哑铃+Kegel锻炼,观察组开展电刺激生物反馈+阴道哑铃+Kegel锻炼。对比两组盆底肌力、尿动力学指标、压力性尿失禁次数以及评分、性生活质量。结果治疗前,两组患者盆底肌力相关指标比较,差异无统计学意义(P>0.05);治疗后,观察组各项盆底肌力相关指标均高于对照组,差异有统计学意义(P<0.05)。治疗前,两组患者尿动力学指标比较,差异无统计学意义(P>0.05);治疗后,与对照组相比,观察组平均尿流率、最大膀胱容量更高,残余尿量更低,差异有统计学意义(P<0.05)。治疗前,两组患者24 h尿失禁次数、ICIQ-SF评分比较,差异无统计学意义(P>0.05);治疗后,观察组24 h尿失禁次数为(1.20±0.25)次、ICIQ-SF评分为(4.02±0.14)分,均低于对照组的(1.68±0.28)次、(6.69±1.08)分,差异有统计学意义(t=8.859、16.986,P<0.05)。治疗前,两组患者FSFI评分、每周性生活次数、每次性生活持续时间比较,差异无统计学意义(P>0.05);治疗后,观察组性生活质量各项指标均高于对照组,差异有统计学意义(P<0.05)。结论对产后盆底功能障碍患者实施电刺激生物反馈+阴道哑铃+Kegel锻炼,能够有效改善患者盆底肌力以及尿动力学,降低压力性尿失禁的发生风险,提升患者的性生活质量。

Objective To investigate the value of electrical stimulation biofeedback+vaginal dumbbell Kegel exercise application in the treatment of postpartum pelvic floor dysfunction.Methods 96 patients with postpartum pelvic floor dysfunction at Zhangzhou Second Hospital from January 2021 to June 2022 were conveniently selected and randomly divided into a control group and an observation group,with 48 patients in each group.The control group carried out vaginal dumbbell+Kegel exercise,and the observation group carried out electrical stimulation biofeedback+vaginal dumbbell+Kegel exercise.Compared the pelvic floor muscle strength,urodynamic indicators,frequency and score of stress urinary incontinence,and quality of sexual life between the two groups.Results Before treatment,there was no statistically significant difference in pelvic floor muscle strength related indicators between the two groups of patients(P>0.05);after treatment,all pelvic floor muscle strength related indicators in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in urodynamic indicators between the two groups of patients(P>0.05);after treatment,the average urine flow rate and maximum bladder volume in the observation group were higher,and the residual urine volume was lower than that in the control group,the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in the number of 24-hour urinary incontinence and ICIQ-SF scores between the two groups of patients(P>0.05);after treatment,the number of urinary incontinence episodes at 24 hours were(1.20±0.25)times and ICIQ-SF score(4.02±0.14)points in the observation group,which were lower than the control group's(1.68±0.28)times and(6.69±1.08)points,the difference was statistically significant(t=8.859,16.986,P<0.05).Before treatment,there was no statistically significant difference in FSFI score,weekly sexual activity frequency,and duration of each sexual activity between the two groups of patients(P>0.05);after treatment,all indicators of sexual life quality in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05).Conclusion The implementation of electrical stimulation biofeedback+vaginal dumbbell+Kegel exercise in patients with postpartum pelvic floor dysfunction can effectively improve patients'pelvic floor muscle strength as well as urodynamics,reduce the risk of stress urinary incontinence,and improve the quality of patients'sexual life.
