睾丸穿刺取精及手淫射出精子行ICSI治疗结局的比较分析

Comparative Analysis of the Outcome of ICSI Treatment for Testicular Puncture and Ejaculation Sperm by Masturbation

目的比较睾丸穿刺取精及手淫射出精子进行卵胞浆内单精子注射(intracytoplasmic sperm injection,ICSI)的临床结局。方法方便选取2020年3月—2021年3月来广东医科大学顺德妇女儿童医院进行治疗的不育症患者332例作为研究对象。根据取精方式的不同分为睾丸穿刺取精组(A组,n=64)和手淫射出精子组(B组,n=268),女方常规超排卵和取卵,成熟卵子行ICSI注射,比较两组患者的正常受精率、正常卵裂率、可利用胚胎率、优质胚胎率、临床妊娠率。结果两组正常受精率、正常卵裂率、可利用胚胎率、优质胚胎率,差异无统计学意义(P>0.05),A组临床妊娠率73.44%高于B组57.46%,差异有统计学意义(χ^(2)=5.519,P=0.019)。结论对于不育的男性能够通过一些技术孕育下一代,对不育男性采用睾丸中取出精子的方式可以有效改善患者不育现状,帮助患者繁育下一代,在临床治疗不育症中效果良好。

Objective To compare the clinical outcomes of intracytoplasmic single sperm injection(ICSI)between testicular puncture and ejaculated sperm by masturbation.Methods A total of 332 infertile patients who received treatment in Shunde Women and Children's Hospital of Guangdong Medical University from March 2020 to March 2021 were conveniently selected as subjects.According to different methods of sperm extraction,they were divided into testis puncture group(group A,n=64)and masturbation ejaculation sperm group(group B,n=268).The woman underwent routine superovulation and ovulation,and the mature ovum underwent ICSI injection.The normal fertilization rate,normal cleavage rate,available embryo rate,high-quality embryo rate and clinical pregnancy rate were compared between the two groups.Results There was no statistically significant difference in the normal fertilization rate,normal cleavage rate,available embryo rate,high-quality embryo rate in two groups(P>0.05).The clinical pregnancy rate of the group A was 73.44%,which was higher than that of the group B(57.46%),and the difference was statistically significant(χ^(2)=5.519,P=0.019).Conclusion For infertile men,the next generation can be conceived through some technologies.The method of removing sperm from testicles for infertile men can effectively improve the status quo of infertility in patients and help patients to breed the next generation,which has a good effect in the clinical treatment of infertility.
