基于医联体的社区康复医学科学科建设探索--以徐汇区枫林街道社区卫生服务中心为例

Exploration of the construction of community rehabilitation medical science department based on medical consortium-taking Fenglin Community Health Service Center of Xuhui District as an example

我国社区康复医学科目前仍处于发展初级阶段,社区卫生服务中心与综合性医疗机构的双向转诊渠道不通畅,康复服务连续性较差。社区卫生服务中心受康复设备落后、专业人才缺乏、技术服务能力不足等问题的限制,难以完全满足患者的康复服务需求,导致社区康复服务供需矛盾愈发凸显。为进一步深化社区卫生综合改革,逐步实现分级诊疗,多地开始探索医疗联合体模式。上海市徐汇区枫林街道社区卫生服务中心于2016年加入“徐汇区-中山医院”医疗联合体,本文基于该中心的实践经验总结社区康复医学科建设情况,为社区康复医学科的学科发展提供参考。

The community rehabilitation medicine in China is still in the primary stage of development.The twoway referral channel between the community health service centers and comprehensive medical institutions is obstructed and the rehabilitation services are not continuous.Due to the limitation of problems such as outdated rehabilitation equipment,lack of professional talents,and poor technical service capacity,it is difficult for the community health service centers to completely meet the rehabilitation service demands of patients so that the contradictions between the supply and demand of community rehabilitation services are more prominent.To further deepen the comprehensive reform of community health and gradually realize the hierarchical diagnosis and treatment,the exploration of the medical consortium mode starts in many places.Fenglin Community Health Service Center of Xuhui District,Shanghai joined the medical consortium of“Xuhui District-Zhongshan Hospital”in 2016.This paper summarizes the construction of the community rehabilitation medicine department based on the practical experience of the center and provides a reference for the discipline development of the community rehabilitation medicine.
