基于CTA对椎动脉发育不良椎动脉形态学的研究

摘要 目的基于CTA技术对纳入研究对象的椎动脉(VA)形态学进行观察评估,并按椎动脉管径进行分组,以期获取椎动脉发育不良(VHA)患者的椎动脉形态学特征及变化特点,从而为本病的进一步研究提供帮助。方法回顾性分析142例符合研究标准受检者的头颈CTA图像,并根据双侧的VA直径将受检者划分为椎动脉发育不良组(n=38)、非等势椎动脉组(n=63)和等势椎动脉组(n=41),以观察各组椎动脉的形态学相关信息(管腔直径、血管长度及迂曲指数),归纳VAH患者椎动脉形态学的特征及变化特点。结果椎动脉发育不良组发育不良侧的VA直径较正常细,差异有统计学意义(P<0.05)。椎动脉发育不良组发育不良侧的VA直径较非等势组劣势侧和等势组略细侧明显变细,差异均有统计学意义(P<0.05)。椎动脉发育不良组发育不良侧的VA长度及迂曲指数较正常侧小,差异均有统计学意义(P<0.05)。椎动脉发育不良组发育不良侧的VA长度及迂曲指数与非等势组劣势侧比较,差异均无统计学意义(P>0.05)。椎动脉发育不良组发育不良侧的VA长度与等势组略细侧比较,差异均无统计学意义(P>0.05);椎动脉发育不良组发育不良侧的VA迂曲指数较等势组略细侧小,差异有统计学意义(P<0.05)。结论基于CTA技术可以准确获取VAH患者的椎动脉相关形态学特征及变化特点,从而为本病的进一步研究提供帮助。 Objective In this study,the morphology of the vertebral artery were observed and analyzed by using computed tomography angiography technology,and we classify the groups according to bilateral vertebral artery diameter,to observe the morphological characteristics of vertebral artery hypoplasia(VAH),thus providing help for further research of this disease.Methods The head and neck computed tomography angiography images of 142 patients the research criteria were retrospectively analyzed,and they were divided into three groups according to bilateral vertebral artery diameter,the three groups include vertebral artery hypoplasia group(n=38),non-equipotentia vertebral artery group(n=63)and equipotentia vertebral artery group(n=41),to observe and compare vertebral artery morphological parameters(lumen diameter,vascular length,tortuosity),and obtain the morphological changes of the vertebral artery in vertebral artery hypoplasia patients.Results The VA diameter of the hypoplasia side of the vertebral artery hypoplasia group was thinner than that of the normal side of the vertebral artery hypoplasia group,the difference was statistically significant(P<0.05).The VA diameter of the hypoplasia side of the vertebral artery hypoplasia group was significantly thinner than that of the inferior side of nonequipotentia vertebral artery group and the slender side of equipotentia vertebral artery group,the differences were statistically significant(P<0.05).The VA length and tortuosity index of the hypoplasia side of the vertebral artery hypoplasia group were smaller than those of the normal side of the vertebral artery hypoplasia group,the differences were statistically significant(P<0.05).The VA length and tortuosity index of the hypoplasia side of the vertebral artery hypoplasia group were not significantly different from those of the inferior side of the non-equipotentia vertebral artery group(P>0.05).The VA length of the hypoplasia side of the vertebral artery hypoplasia group was not significantly different from those of the slender side of the equipotentia vertebral artery group(P>0.05).The VA tortuosity index of the hypoplasia side of the vertebral artery hypoplasia group was smaller than that of the slender side of the equipotentia vertebral artery group,the difference was statistically significant(P<0.05).Conclusion Based on the CTA technology,the morphological characteristics and changes of the vertebral artery in patients with VAH can be accurately obtained,thus providing help for further research on this disease.
