分化型甲状腺癌患者妊娠期间激素水平变化及其对疾病发生发展影响的研究进展 被引量:4

摘要 分化型甲状腺癌(DTC)是甲状腺恶性肿瘤最常见的病理类型,且好发于育龄期女性。女性妊娠期间高水平雌孕激素和人绒毛膜促性腺激素(hCG)不仅能影响甲状腺激素水平,还可通过不同途径对DTC的发生发展产生影响。雌孕激素对DTC细胞增殖、凋亡、侵袭和转移方面的影响主要有基因途径和非基因途径。雌激素通过促进周期蛋白D1 (Cyclin D1)的表达和激活丝裂原活化蛋白激酶(MAPK)信号通路促进DTC细胞增殖,通过雌激素受体α(ERα)诱导B细胞淋巴瘤2(Bcl-2)蛋白的产生抑制DTC细胞凋亡,通过下调β-连环蛋白表达和刺激甲状腺癌细胞分泌血管内皮生长因子(VEGF)促进DTC细胞侵袭和转移。hCG和促甲状腺激素(TSH)及其受体的结构具有同源性,二者功能方面相互影响,对肿瘤的侵袭和转移亦有一定影响。现对DTC患者妊娠期间激素水平变化及其对DTC发生发展的影响进行简要综述,为妊娠期女性甲状腺癌的防治提供理论依据。
