早期运动康复在急性ST段抬高型心肌梗死患者急诊介入术后的效果

Effect of Early Exercise Rehabilitation on Patients with Acute ST Segment Elevation Myocardial Infarction After Emergency Intervention

目的探讨早期运动康复在急诊介入术后急性ST段抬高型心肌梗死(STEMI)患者中的应用效果。方法选取2021年12月1日-2022年5月30日在安庆市立医院行急诊经皮冠状动脉介入(PCI)术后的急性STEMI患者60例,采用随机数字表法分为康复组和对照组,每组30例。康复组予以早期运动康复与药物治疗,对照组仅予以药物治疗。比较两组6分钟步行试验(6MWT)距离、心脏超声左室射血分数(LVEF)值、左室舒张末期内径(LVDd)、生活满意度量表评分、心率。结果两组出院前6MWT距离比较,差异无统计学意义(P>0.05);康复组1个月后门诊复查时LVEF值、6MWT距离、生活满意度量表评分均高于对照组,差异有统计学意义(P<0.05);康复组1个月后门诊复查时心率低于对照组,差异有统计学意义(P<0.05);两组1个月后门诊复查时LVDd比较,差异无统计学意义(P>0.05)。结论早期运动康复有利于急性心肌梗死患者心脏功能的恢复,可稳定心率,提高运动耐量及生活质量,值得临床应用。

Objective To explore the application effect of early exercise rehabilitation in patients with acute ST-segment elevation myocardial infarction(STEMI)after emergency intervention.Methods Sixty patients with acute STEMI after emergency percutaneous coronary intervention(PCI)in Anqing Municipal Hospital from December 1,2021 to May 30,2022 were randomly divided into rehabilitation group and control group,with 30 patients in each group.The rehabilitation group was given early exercise rehabilitation and drug treatment,while the control group was only given drug treatment.The 6-minute walking test(6MWT)distance,left ventricular ejection fraction(LVEF)value,left ventricular end-diastolic diameter(LVDd),life satisfaction scale score and heart rate were compared between the two groups.Results There was no significant difference in 6MWT distance between the two groups before discharge(P>0.05).The LVEF value,6MWT distance and life satisfaction scale score of the rehabilitation group were higher than those of the control group after 1 month of outpatient review,and the difference was statistically significant(P<0.05).After 1 month of outpatient review,the heart rate of the rehabilitation group was lower than that of the control group,and the difference was statistically significant(P<0.05).There was no significant difference in LVDd between the two groups after 1 month of outpatient review(P>0.05).Conclusion Early exercise rehabilitation is beneficial to the recovery of cardiac function in patients with acute myocardial infarction,which can stabilize heart rate,improve exercise tolerance and quality of life,and is worthy of clinical application.
