股骨颈骨折患者术后发生创伤后应激障碍的相关危险因素分析

Analysis of risk factors for post-traumatic stress disorder in patients with femoral neck fracture

目的探究股骨颈骨折患者术后发生创伤后应激障碍(PTSD)的相关危险因素。方法选取2020年9月至2022年6月南昌大学第二附属医院骨科收治的80例股骨颈骨折患者,采用创伤后应激障碍量表平民版(PCL-C)评估患者术后1周的PTSD发生状况,并收集相关基线资料,分析股骨颈骨折患者术后发生PTSD的相关危险因素。结果80例股骨颈骨折患者中,有25例患者术后发生PTSD,发生率为31.25%。性别、心理弹性、性格特征、预计康复结局与股骨颈骨折患者术后发生PTSD有关(P<0.05);经Logistic回归分析显示,女性、内向、预计康复结局为残疾是股骨颈骨折患者术后发生PTSD的危险因素(OR>1,P<0.05);心理弹性评分较高是股骨颈骨折患者术后发生PTSD的保护因素(OR<1,P<0.05)。结论股骨颈骨折患者术后发生PTSD与患者为女性、心理弹性较差、内向、预计康复结局为残疾等因素有关,临床可采取相应措施来降低PTSD发生率,促进患者康复。

Objective To explore the risk factors of post-traumatic stress disorder(PTSD)in patients with femoral neck fracture after surgery.Methods A total of 80 patients with femoral neck fracture admitted to the Department of Orthopedics of the Second Affiliated Hospital of Nanchang University from September 2020 to June 2022 were selected.The occurrence of PTSD in patients 1 week after surgery was assessed by using the civilian version of post-traumatic Stress Disorder Scale(PCL-C),and relevant baseline data were collected to analyze the risk factors of PTSD in patients with femoral neck fracture after surgery.Results Among 80 patients with femoral neck fracture,25 patients developed PTSD after operation,with an incidence of 31.25%.Gender,mental resilience,personality traits and expected recovery outcome were correlated with PTSD after femoral neck fracture(P<0.05).Logistic regression analysis showed that female,introversion and disability were risk factors for postoperative PTSD in patients with femoral neck fracture(OR>1,P<0.05).A higher psychological resilience score was a protective factor for postoperative PTSD in patients with femoral neck fracture(OR<1,P<0.05).Conclusion Postoperative PTSD in patients with femoral neck fracture is related to female patients,poor mental resilience,introverted,disability and other factors.Clinical measures can be taken to reduce the incidence of PTSD and promote the rehabilitation of patients.
