经肠镜下支架置入联合腹腔镜微创治疗结直肠癌并肠梗阻临床疗效

Clinical Effect of Endoscopic Stent Implantation Combined with Laparoscopic Minimally Invasive Treatment for Colorectal Cancer with Intestinal Obstruction

目的:分析经肠镜下支架置入联合腹腔镜微创治疗结直肠癌并肠梗阻临床疗效。方法:选取2021年3月~2022年10月天津北大医疗海洋石油医院收治的64例结直肠癌并肠梗阻患者为研究对象,基于治疗方式不同将其分为对照组和观察组,对照组行开腹手术加术中结肠灌洗进行治疗,观察组行经肠镜下支架置入联合腹腔镜进行治疗,观察两组患者的手术指标和手术后指标。结果:观察组手术时间明显短于对照组(P<0.05),观察组术中出血量和手术切口长度均显著小于对照组(P<0.05),观察组淋巴结清扫数量显著多于对照组(P<0.05)。观察组术后肛门排气时间和住院时间均显著短于对照组(P<0.05),观察组并发症发生率显著低于对照组(P<0.05),观察组治疗费用高于对照组,但是组间差异无统计学意义(P>0.05)。结论:经肠镜下支架置入联合腹腔镜微创治疗结直肠癌并肠梗阻,临床疗效显著。

Objective:To analyze the clinical effect of endoscopic stent placement combined with laparoscopic minimally invasive treatment for colorectal cancer with intestinal obstruction.Methods:64 patients with colorectal cancer and intestinal obstruction admitted to PKU Care CNOOC Hospital,Tianjin from March 2021 to October 2022 were selected as the study subjects.They were divided into control group and observation group based on different treatment methods.The control group was treated with open surgery and intraoperative colon lavage,while the observation group was treated with endoscopic stent placement combined with laparoscopy,and the operative and postoperative indexes of the two groups were observed.Results:The operation time in the observation group was significantly shorter than that in the control group(P<0.05).The amount of intraoperative bleeding and the length of surgical incision in the observation group were significantly smaller than those in the control group(P<0.05).The number of lymph node dissection in the observation group was significantly greater than that in the control group(P<0.05).The postoperative anal exhaust time and hospital stay in the observation group were significantly shorter than those in the control group(P<0.05),the complication rate in the observation group was significantly lower than that in the control group(P<0.05),and the treatment cost in the observation group was higher than that in the control group,but there was no significant difference between the groups(P>0.05).Conclusion:Endoscopic stent implantation combined with laparoscopic minimally invasive treatment of colorectal cancer with intestinal obstruction has a significant clinical effect.
