彩色多普勒超声在健康人群乳腺癌筛查中的诊断价值分析

Diagnostic Value of Color Doppler Ultrasound in Screening Breast Cancer in Healthy People

目的:探讨彩色多普勒超声在健康人群乳腺癌筛查中的诊断价值。方法:从本院接受乳腺癌健康体检的受检者中选取400例,时间为2022年1月~2022年12月。受检者接受超声检查、乳腺触诊检查,比较两种方式乳腺病变(增生、结节、肿块)的检出情况。检出乳腺异常的患者实施乳腺钼靶的进一步检查结果,并以患者的最终诊断作为金标准,计算超声、乳腺钼靶在乳腺癌筛查中的灵敏度等指标。结果:超声检查结果在乳腺异常(增生、结节、肿块)中的检出率显著高于乳腺触诊,对比差异显著(P<0.05);其中超声乳腺增生检出43例,乳腺结节检出42例,乳腺肿块37例。对122例乳腺异常患者实施乳腺钼靶检查,并与超声诊断结果对比。超声检查结果显示:在乳腺癌诊断中的灵敏度为75.00%(12/16),特异度为89.62%(95/106),符合率为87.70%(107/122)。乳腺钼靶结果显示:在乳腺癌的诊断中灵敏度为87.50%(14/16),特异度为96.23%(102/106),符合率为95.08%(116/122)。乳腺超声、乳腺钼靶在乳腺癌诊断中的灵敏度、特异度对比差异不显著(P>0.05),符合率中钼靶高于超声诊断(P<0.05)。结论:彩色多普勒超声在健康人群乳腺癌筛查中可发挥令人满意的效果,相较乳腺触诊更容易发现乳腺病变,并且与乳腺钼靶相比较,在乳腺癌诊断中的灵敏度、特异度无显著差异,具有良好的诊断效能,能够为乳腺癌的早期诊治提供可靠的诊断依据。

Objective:To explore the diagnostic value of color Doppler ultrasound in screening breast cancer in healthy people.Methods:The application value of ultrasound in breast cancer screening of healthy people was analyzed.400 patients were selected from the breast cancer health examination in our hospital,and the time was from January 2022 to December 2022.The subject underwent ultrasound and breast palpation,compare the detection of breast lesions(hyperplasia,nodule and mass)in two ways.The patients with breast abnormalities were further examined with mammography,and the final diagnosis of the patients was taken as the gold standard to calculate the sensitivity of ultrasound and mammography in breast cancer screening and other indicators.Results:The detection rate of ultrasound results in breast abnormalities(hyperplasia,nodule and mass)was significantly higher than that of breast palpation,the difference was significant(P<0.05);Among them,43 cases of breast hyperplasia,42 cases of breast nodules and 37 cases of breast masses were detected by ultrasound.122 patients with abnormal breast were examined by mammography and compared with the results of ultrasound diagnosis.The results of ultrasonography showed that the sensitivity of breast cancer diagnosis was 75.00%(12/16),the specificity was 89.62%(95/106),and the coincidence rate was 87.70%(107/122).The breast molybdenum target results showed that the sensitivity in the diagnosis of breast cancer was 87.50%(14/16),the specificity was 96.23%(102/106),and the coincidence rate was 95.08%(116/122).There was no significant difference in sensitivity and specificity between breast ultrasound and breast molybdenum target in the diagnosis of breast cancer(P>0.05).Conclusion:Color Doppler ultrasound can play a satisfactory role in screening breast cancer in healthy people.Compared with breast palpation,it is easier to find breast lesions.Compared with breast molybdenum target,it has no significant difference in sensitivity and specificity in the diagnosis of breast cancer.It has good diagnostic efficacy and can provide reliable diagnostic basis for early diagnosis and treatment of breast cancer.
