肌骨超声检查在急性痛风性关节炎诊断及病情评估中的价值

The Value of Musculoskeletal Ultrasound in the Diagnosis and Evaluation of Acute Gouty Arthritis

目的:浅析肌骨超声检查对于急性痛风性关节炎诊断价值以及对病情评估的作用。方法:将2021年5月~2022年11月来本院治疗急性痛风性关节炎患者111例设为实验组,再取同期来本院治疗其他类型关节炎患者100例设为对照组。两组患者全部接受肌骨超声检查,对比两组关节炎患者肌骨超声检查声像图表现、肌骨超声评分以及肌骨超声的诊断效能。结果:在肌骨超声表现比较中,实验组患者双轨征检出率、痛风石检出率、高回声点检出率、骨侵蚀检出率、腱鞘炎检出率均存在明显差异性,其中,实验组双轨征和痛风石以及高回声点表现高于对照组(P<0.05),骨侵蚀和腱鞘炎表现低于对照组(P<0.05),数据存在统计学意义;实验组患者在肌骨超声分数比较中,滑膜病损分数、血流信号分数、骨侵蚀分数、关节囊物质沉积分数和综合分数较对照组患者各项分数更高(P<0.05),数据存在统计学意义。结论:对急性痛风性关节炎患者实施肌骨超声检查,对患者病情诊断和病情评估之间有显著意义,可靠性更高。

Objective:To analyze the diagnostic value of musculoskeletal ultrasound in acute gouty arthritis and its role in disease evaluation.Methods:The patients who came to our hospital to treat acute gouty arthritis from May 2021 to November 2022 were taken as observation patients for research.There were 111 patients in total,which were set as the experimental group.Then,the patients who came to our hospital to treat other types of arthritis during this period were set as the control group,with 100 patients in total.All patients in the two groups received musculoskeletal ultrasound examination.The sonographic manifestations,musculoskeletal ultrasound scores,and diagnostic efficacy of musculoskeletal ultrasound were compared between the two groups.Results:In the comparison of musculoskeletal ultrasonic manifestations,there were significant differences in the detection rate of double track sign,gout stone,hyperechoic spot,bone erosion and tenosynovitis in the experimental group.Among them,the double track sign,gout stone and hyperechoic spot in the experimental group were higher than those in the control group(P<0.05),while the bone erosion and tenosynovitis were lower than those in the control group(P<0.05).The data were statistically significant;In the comparison of musculoskeletal ultrasound scores of patients in the experimental group,the scores of synovial lesions,blood flow signals,bone erosion,joint capsule material deposition,and comprehensive scores were higher than those of patients in the control group(P<0.05).Conclusion:The musculoskeletal ultrasound examination in patients with acute gouty arthritis has significant significance in the diagnosis and evaluation of the patient’s condition,and has higher reliability.
