动态心电图与常规心电图诊断冠心病患者心肌缺血与心律失常的效果对比研究

Comparative Study on the Effect of Dynamic Electrocardiogram and Conventional Electrocardiogram in Diagnosis of Myocardial Ischemia and Arrhythmia in Patients with Coronary Heart Disease

目的:研究动态心电图(AECG)诊断冠心病患者心肌缺血与心律失常的实际价值。方法:从本院2021年1月~2022年6月收治的冠心病患者中随机抽取100例作为研究对象,全部患者先后接受了常规心电图检查与AECG,其中,将常规心电图检查设为常规组,将AECG检查设为动态组,对比两组心肌缺血阳性检出率、心律失常检出率、ST段抬高检出率与ST段压低检出率等情况。结果:比较心肌缺血阳性检出率,动态组为88.0%,远比常规组的54.0%更高;比较心律失常检出率,动态组为83.0%,远比常规组的60.0%更高,P<0.05,差异具有统计学意义。相较于常规组,动态组的ST段抬高检出率与ST段压低检出率均更高(P<0.05),差异存在统计学意义。比较室内早搏、房性逸搏、房性期前收缩、心房颤动、室性心动过速、短阵室上速等指标,两组检出结果差异不大(P>0.05),差异无统计学意义,但是动态组检出的例数比常规组多。结论:临床检测冠心病患者是否有心肌缺血与心律失常可选择动态心电图,不仅操作简单,安全可靠,而且检出效果好,为冠心病的临床诊疗提供了可靠的依据。

Objective:To observe the practical value of dynamic electrocardiogram(AECG)in diagnosing myocardial ischemia and arrhythmia in patients with coronary heart disease.Methods:From January 2021 to June 2022,100 patients with coronary heart disease treated in our hospital were randomly selected for study.All patients were examined by routine ECG and dynamic ECG.Among them,routine ECG was set as routine group and dynamic ECG was set as dynamic group.The positive detection rate of myocardial ischemia,arrhythmia,ST segment elevation and ST segment elevation were compared between the two groups.Results:The positive detection rate of myocardial ischemia was 88.0%in the dynamic group,which was much higher than 54.0%in the conventional group;Compared with the detection rate of arrhythmia,the dynamic group was 83.0%,which was much higher than the 60.0%of the conventional group.Because of P<0.05,the difference between the above groups was statistically significant.Compared with the conventional group,the detection rate of ST segment elevation and ST segment depression in the dynamic group were higher(P<0.05),and the difference was statistically significant.Compared with the indexes such as ventricular premature beats,atrial escape beats,atrial premature contractions,atrial fibrillation,ventricular tachycardia,short supraventricular tachycardia,there was no significant difference between the two groups(P>0.05),but the number of cases detected in the dynamic group was more than that in the control group.Conclusion:Dynamic electrocardiogram can be used to detect whether there is myocardial ischemia and arrhythmia in patients with coronary heart disease.It is not only simple,safe and reliable,but also has good detection effect,providing a reliable basis for clinical diagnosis and treatment of coronary heart disease.
