经鼻高流量湿化治疗仪结合支气管镜肺泡灌洗用于重症肺炎并低氧血症患者的价值

The Value of Nasal High Flow Humidification Therapy Instrument Combined with Bronchoscopic Alveolar Lavage in Patients with Severe Pneumonia and Hypoxemia

目的:探究重症肺炎并低氧血症患者采用经鼻高流量湿化治疗仪结合支气管镜肺泡灌洗治疗的应用效果。方法:依据随机数字表法对本院2020年2月~2022年2月收治的70例重症肺炎并低氧血症患者分组(各组35例)探究,对照组采用支气管镜肺泡灌洗治疗,观察组同时加用经鼻高流量湿化治疗仪治疗,观察两组炎性因子水平、血气指标、低氧血症发生率、APACHEⅡ评分。结果:治疗前,两组炎性因子水平、血气指标、APACHEⅡ评分对比无差异(P>0.05);治疗后,两组对比,观察组炎性因子水平更低,数据差异明显(P<0.05);治疗后,两组血气指标水平对比有差异(P<0.05);两组对比,观察组低血氧症发生率、APACHEⅡ评分更低,数据对比有差异(P<0.05)。结论:经鼻高流量湿化治疗仪结合支气管镜肺泡灌洗可显著减轻重症肺炎并低氧血症患者炎症反应,改善血气指标,加快机体恢复。

Objective:To explore the application effect of nasal high flow humidification therapy instrument combined with bronchoscopic alveolar lavage in the treatment of severe pneumonia with hypoxemia.Methods:70 patients with severe pneumonia and hypoxemia admitted to our hospital from February 2020 to February 2022 were divided into groups(35 in each group)according to the method of random number table.The control group was treated with bronchoscopic alveolar lavage,and the observation group was also treated with nasal high flow humidification therapy instrument.The levels of inflammatory factors,blood gas indicators,incidence of hypoxemia,and APACHE II scores were observed in the two groups.Results:Before treatment,there was no difference in the levels of inflammatory factors,blood gas index and APACHE II score between the two groups(P>0.05);After treatment,compared with the two groups,the level of inflammatory factors in the observation group was lower,and the data difference was significant(P<0.05);After treatment,there was a difference in blood gas index between the two groups(P<0.05);Compared with the two groups,the incidence of hypoxemia and APACHE II score in the observation group were lower,and the data were different(P<0.05).Conclusion:Transnasal high flow humidification therapy instrument combined with bronchoscopic alveolar lavage can significantly reduce the inflammatory reaction of patients with severe pneumonia and hypoxemia,improve the blood gas index,and accelerate the recovery of the body.
