艾司氯胺酮预处理臂丛神经阻滞在老年粉碎性肱骨近端骨折手术患者中的应用

Application of Esketamine Pretreatment of Brachial Plexus Block in Elderly Patients with Comminuted Proximal Humeral Fracture Surgery

目的探讨艾司氯胺酮预处理臂丛神经阻滞在老年粉碎性肱骨近端骨折手术患者中的应用效果。方法76例老年粉碎性肱骨近端骨折手术患者随机分为两组。两组均接受臂丛神经阻滞麻醉,A组予以艾司氯胺酮预处理,B组予以同等容量生理盐水预处理。比较两组的围手术期指标、术后疼痛程度及不良反应。结果A组的麻醉药物使用剂量低于B组,定向力恢复时间、苏醒时间均短于B组,术后VAS评分、不良反应总发生率均低于B组(P<0.05)。结论艾司氯胺酮预处理臂丛神经阻滞可显著减少老年粉碎性肱骨近端骨折手术患者的麻醉药物使用剂量,缩短苏醒时间,缓解术后疼痛,降低不良反应发生率。

Objective To explore the application effect of esketamine pretreatment of brachial plexus block in elderly patients with comminuted proximal humeral fracture surgery.Methods 76 elderly patients with comminuted proximal humeral fracture surgery were randomly divided into two groups.Both groups received brachial plexus block anesthesia.Group A received esketamine pretreatment,and group B received the same volume of normal saline solution pretreatment.Perioperative indicators,postoperative pain degree and adverse reactions were compared between the two groups.Results The dosage of anesthetic drugs in group A was lower than that in group B,the recovery time of orientation and awakening time were shorter than those in group B,the postoperative VAS scores and the total incidence of adverse reactions were lower than those in group B(P<0.05).Conclusions Esketamine pretreatment of brachial plexus block can significantly reduce the dosage of anesthetic drugs in elderly patients with comminuted proximal humeral fracture surgery,shorten awakening time,relieve postoperative pain,and reduce the incidence of adverse reactions.
