PCT、CRP与急性心肌梗死并发心力衰竭患者病情严重程度的相关性及对预后的预测价值

Correlations of PCT and CRP with the Severity of Acute Myocardial Infarction Complicated with Heart Failure and Predictive Value for Prognosis

目的探讨PCT、CRP与急性心肌梗死并发心力衰竭患者病情严重程度的相关性及对预后的预测价值。方法90例急性心肌梗死并发心力衰竭患者均进行PCT、CRP以及心功能指标检测,比较不同心功能分级患者的PCT、CRP、LVEF、LVSV水平,分析PCR、CRP与LVEF、LVSV、心功能分级的相关性,评估PCT、CRP对不良预后的预测价值。结果不同心功能分级患者的PCT、CRP、LVEF、LVSV比较,差异有统计学意义(P<0.05)。PCT、CRP与LVEF呈负相关,与心功能分级呈正相关(P<0.05)。ROC曲线显示,PCT、CRP对急性心肌梗死并发心力衰竭患者预后具有显著的预测价值(AUC=0.847、0.811,P<0.05)。结论PCT、CRP可作为评估急性心肌梗死并发心力衰竭患者病情严重程度及预后的有效指标。

Objective To explore the correlations of PCT and CRP with the severity of acute myocardial infarction complicated with heart failure and predictive value for prognosis.Methods PCT,CRP and cardiac function indicators were detected in 90 patients with acute myocardial infarction complicated with heart failure.The levels of PCT,CRP,LVEF and LVSV in patients with different cardiac function grades were compared.The correlations of PCR and CRP with LVEF,LVSV and cardiac function grade were analyzed.The predictive value of PCT and CRP for poor prognosis was evaluated.Results There were significant differences in PCT,CRP,LVEF and LVSV among patients with different cardiac function grades(P<0.05).PCT and CRP were negatively correlated with LVEF and positively correlated with cardiac function grade(P<0.05).ROC curve showed that PCT and CRP had significant predictive value for the prognosis of patients with acute myocardial infarction complicated with heart failure(AUC=0.847,0.811;P<0.05).Conclusions PCT and CRP can be used as effective indicators to evaluate the disease severity and prognosis of patients with acute myocardial infarction complicated with heart failure.
