CT及MRI在踝关节骨折患者分型、分度及预后评估中的应用

摘要 目的:分析电子计算机断层扫描(Computed Tomography,CT)、磁共振成像(Magnetic Resonance Imaging,MRI)诊断踝关节骨折患者中的分型、分度、预后价值。方法:选择我院骨科2017年8月-2018年10月收治治疗的踝关节骨折患者92例,全部患者均进行CT、MRI诊断,分析分型评估、疗效评估结果。结果:分型评估方面,92例踝关节骨折患者中包括旋后外旋型、旋前外旋型、旋后内收型、旋前外展型、旋前背屈型。经CT后处理可以清晰显示患者的踝关节三踝骨折、周围肌腱损伤等情况,诊断准确率达100.00%;疗效评估方面,全部患者均进行手术治疗,术后良好率达90.21%;预测结果方面,有15例(16.30%)患者发生了创伤性关节炎,随访3年后发生创伤性关节炎8例(8.69%)。结论:CT检查、MRI检查可以进行踝关节骨折准确的分型、分度,助于患者手术与术后关节修复情况评估,提高了患者的安全性与生活质量。 Objective:To analyze the value of computed tomography(CT)and magnetic resonance imaging(MRI)in the diagnosis of ankle fracture.Methods:A total of 92 patients with ankle fracture treated in the orthopaedics department of our hospital from August 2017 to October 2018 were selected.With the approval of the hospital ethics committee and the informed consent of the participating pa-tients and their families,all patients underwent CT and MRI diagnosis.The results of typing evaluation and curative effect evaluation were analyzed.Results:In terms of classification and evaluation,92 cases of ankle fracture included supination external rotation,pronation external rotation,supination adduction,pronation abduction and pronation dorsiflexion.After CT post-processing,the patients,triple an-kle fracture and peripheral tendon injury can be clearly displayed,and the diagnostic accuracy is 100.00%.In terms of efficacy evalua-tion,all patients were treated with surgery,and the postoperative good rate was 90.21%.In terms of prediction results,15 patients(16.30%)developed traumatic arthritis,and 8 patients(8.69%)developed traumatic arthritis after 3 years of follow-up.Conclusion:CT and MRI can accurately classify and grade ankle fracture,help to evaluate the operation and postoperative joint repair,and improve the safety and quality of life of patients.
