女性压力性尿失禁严重程度分布及影响因素分析

Analysis of the factors influencing the severity of female stress urinary incontinence

目的了解压力性尿失禁(SUI)患者尿失禁严重程度分布,分析探讨影响SUI严重程度的因素。方法选取2020年9月—2021年10月在江西省妇幼保健院妇女保健科和妇科就诊的SUI女性493例为研究对象,采用自制SUI流行病学调查问卷收集研究对象的基本情况,采用国际尿失禁咨询委员会尿失禁问卷简表(ICI-Q-SF)收集研究对象尿失禁严重程度。根据ICI-Q-SF评分将研究对象分为轻度SUI组(≤7分)、中度SUI组(8~13分)及重度SUI组(14~21分)。采用t检验和χ^(2)检验对可能影响SUI严重程度的相关变量进行单因素分析,采用logistic回归分析评估各影响因素的优势比(OR)和95%置信区间(CI)。结果共纳入493例SUI女性,其中轻度337例(68.4%)、中度135例(27.4%)及重度21例(4.2%)。多因素logistic回归分析结果显示,年龄、文化程度及BMI与SUI严重程度之间存在显著相关性(均P<0.05)。结论年龄、文化程度及BMI是中重度SUI的独立影响因素,年龄大、文化程度低及肥胖的SUI女性尿失禁症状往往更重,SUI临床诊治中应重点关注此类患者。

Objective To investigate the severity of stress urinary incontinence(SUI)among SUI women and to explore the factors associated with SUI severity.Methods 493 SUI women who visited the Department of Women's Health Care and Department of Gynecology of Jiangxi Provincial Maternal and Child Health Hospital from September 2020 to October 2021 were enrolled.The self-made SUI epidemiological questionnaire was used to collect the basic information of the subjects.The severity of SUI was collected by using the ICI-Q-SF.SUI women were divided into three groups according to the ICI-Q-SF score:mild SUI group(≤7 points),moderate SUI group(8-13 points)and severe SUI group(14-21 points).T test and Chi-square test were used for the univariate analysis of the related variables which might be associated with the severity of SUI logistic regression analysis was applied to evaluate the odds ratio(OR)and 95%confidence interval(CI)of each influencing factor.Results A total of 493 SUI women were included in this study,including 337 cases(68.4%)with mild SUI,135 cases(27.4%)with moderate SUI and 21 cases(4.2%)with severe SUI.The results of multivariate logistic regression analysis showed that age,educational level and BMI were significantly correlated with the severity of SUI(P<0.05).Conclusion Age,education level and BMI were the independent influencing factors of moderate-severe SUI,Older,less educated and obese women with SUI tended to have more severe urinary incontinence symptom,and more attention should be paid to such patients in the clinical diagnosis and treatment of SUI.
