自拟破血化瘀补髓方对高血压脑出血患者的影响

Effects of Self-Made Poxue Huayu Busui Recipe(自拟破血化瘀补髓方)on Patients with Hypertensive Cerebral Hemorrhage

目的探讨自拟破血化瘀补髓方对高血压脑出血患者的影响。方法选取该院2016年12月-2018年12月60例高血压脑出血患者作为研究对象,经随机数表法分为观察组(30例)和对照组(30例),两组均接受微创血肿清除术,对照组术后予以西医常规治疗,观察组在此基础上予以自拟破血化瘀补髓方治疗。比较两组临床疗效、治疗前后中医症状积分、脑血流动力学指标、免疫-炎性反应指标、神经功能、日常生活活动能力、不良反应及预后。结果观察组治疗总有效率为93.33%(28/30)高于对照组73.33%(22/30)(P<0.05);观察组治疗后意识模糊、语言不利及双下肢活动无力等中医主症积分均低于对照组(P<0.05);观察组治疗后脑血管平均流速、脑血管平均流量均高于对照组,血管外周阻力低于对照组(P<0.05);观察组治疗后血清粒细胞巨噬细胞集落刺激因子(GM-CSF)、高迁移率族蛋白1(HMGB-1)均低于对照组(P<0.05);观察组治疗后美国国立研究院卒中量表(NIHSS)评分低于对照组,Barthel指数(BI)高于对照组(P<0.05);两组治疗期间均未出现严重不良反应;随访6个月,观察组格拉斯哥结局量表(GOS)评分高于对照组(P<0.05)。结论自拟破血化瘀补髓方可有效缓解高血压脑出血患者的临床症状,调节脑血流动力学,降低免疫-炎性反应因子水平,促进神经功能恢复,提高日常生活活动能力,改善患者预后。

Objective To explore the effects of Self-made Poxue Huayu Busui Recipe(自拟破血化瘀补髓方)on patients with hypertensive cerebral hemorrhage.Methods A total of 60 patients with hypertensive cerebral hemorrhage in the hospital were enrolled as the research objects between December 2016 and December 2018.According to random number table method,they were divided into the observation group(30 cases)and the control group(30 cases).Both groups underwent minimally invasive evacuation of hematoma.After that,the control group was treated with routine western medicine,while the observation group was treated with Self-made Poxue Huayu Busui Recipe.The clinical curative effect,scores of TCM symptoms,cerebral hemodynamics indexes,immune-inflammatory response indexes,nerve function,activities of daily living,adverse reactions and prognosis before and after treatment were compared between the two groups.Results The total response rate of treatment in the observation group was higher than that in the control group[93.33%(28/30)vs 73.33%(22/30)](P<0.05).After treatment,the scores of main TCM symptoms(clouding of consciousness,unsmooth speech,activity intolerance of lower limbs)in the observation group were lower than those in the control group(P<0.05).After treatment,the cerebrovascular average velocity and flow in the observation group were higher than those in the control group,while the peripheral vascular resistance was lower than that in the control group(P<0.05).After treatment,the levels of serum granulocyte macrophage colony stimulating factor(GM-CSF)and high mobility group box 1(HMGB-1)in the observation group were lower than those in the control group(P<0.05).After treatment,the National Institute of Health stroke scale(NIHSS)score in the observation group was lower than that in the control group,while Barthel index(BI)was higher than that in the control group(P<0.05).There were no serious adverse reactions during treatment in either group.After 6 months of follow-up,the Glasgow Outcome Scale(GOS)score in the observation group was higher than that in the control group(P<0.05).Conclusion Self-made Poxue Huayu Busui Recipe can effectively relieve the clinical symptoms in patients with hypertensive cerebral hemorrhage,regulate the cerebral hemodynamics,reduce the levels of immune-inflammatory response factors,promote the recovery of nerve function and improve the activities of daily living and prognosis.
