黄葵胶囊治疗早期糖尿病肾病的疗效及机制分析

Analysis on Effect and Mechanism of Huangkui Capsule(黄葵胶囊) in Treatment of Early Diabetic Nephropathy

目的 分析黄葵胶囊治疗早期糖尿病肾病(DN)的疗效及机制。方法 将2016年1月—2020年5月医院收治的100例早期DN患者随机分为对照组和观察组。两组均进行常规治疗,在此基础上对照组采用马来酸依那普利片进行治疗,观察组在对照组的基础上采用黄葵胶囊进行治疗,两组均治疗8周。统计两组治疗8周后的疗效,比较两组治疗前和治疗8周后血清炎性因子水平、肾功能及胰岛素抵抗。结果 治疗8周后,观察组总有效率为94.00%,显著高于对照组80.00%(P<0.05)。治疗8周后,两组血清超敏C反应蛋白(hs-CRP)、白细胞介素-1(IL-1)、肿瘤坏死因子-α(TNF-α)、β2-微球蛋白(β2-MG)、胱抑素C(Cys-C)、胰岛素(FIN)水平及尿微量白蛋白/尿肌酐(UACR)、稳态模型评估IR指数(HOMA-IR)低于治疗前,观察组低于对照组(P<0.05)。结论 黄葵胶囊联合马来酸依那普利片治疗早期DN患者,可有效改善患者肾功能,具有较好的治疗效果,其治疗机制可能与降低机体炎症反应和胰岛素抵抗有关。

Objective To analyze the effect and mechanism of Huangkui Capsule(黄葵胶囊)in the treatment of early diabetic nephropathy(DN).Methods A total of 100 patients with early DN admitted to the hospital from January 2016 to May 2020 were selected as the research objects,and were divided into the control group(50 cases)and the observation group(50 cases).Both groups received the conventional treatment.On this basis,the control group was treated with enalapril maleate tablets and the observation group was treated with Huangkui Capsule on the basis of the control group.Both groups were treated for 8 weeks.The curative effect of the two groups after 8 weeks of treatment was counted and the levels of serum inflammatory factors,renal function and insulin resistance were compared between the two groups before and after 8 weeks of treatment.Results After 8 weeks of treatment,the total effective rate in the observation group was higher than the control group(P<0.05).After 8 weeks of treatment,the levels of serum hypersensitivity C-reactive protein(hs-CRP),interleukin-1(IL-1),tumor necrosis factor-α(TNF-α),cystatin C(Cys-C),insulin(FIN),urinary microalbumin/urinary creatinine(UACR)and homeostasis model assessment IR index(HOMA-IR)in both groups lower than those before treatment and those of the observation group were lower than those of the control group(P<0.05).Conclusion Huangkui Capsule combined with enalapril maleate tablets can effectively improve the renal function in patients with early DN,and has a good therapeutic effect.Its therapeutic mechanism may be related to the reduction of inflammatory response and insulin resistance.
