甲状腺癌患者术后心理弹性状况及影响因素

Psychological Resilience of Postoperative Patients with Thyroid Cancer and Its Influencing Factor

目的 探讨甲状腺癌(TC)患者术后心理弹性状况及其影响因素。方法 选取2019年5月至2022年7月郑州大学第一附属医院收治的115例术后TC患者作为研究对象,采用心理弹性量表(CD-RISC)评估其心理状态,多元线性回归分析TC患者术后CD-RISC评分的影响因素。结果 115例TC患者术后CD-RISC评分为(57.67±4.67)分,低于国内常模[(65.40±13.90)分](P<0.05)。不同受教育程度、肿瘤分期、自我效能感量表(GSES)评分、家庭功能、大五人格问卷-神经质分量表(FFI-N)评分在患者的CD-RISC评分方面比较,差异有统计学意义(P<0.05)。受教育程度、肿瘤分期、GSES评分、家庭功能、FFI-N评分是TC患者CD-RISC评分的影响因素,差异有统计学意义(P<0.05)。结论 TC患者术后心理弹性水平普遍较差,其主要影响因素为受教育程度、肿瘤分期、GSES评分、家庭功能、FFI-N评分,需采取针对性措施提高TC患者心理弹性,建立积极应对方式,促进其身心健康发展,改善生活质量。

Objective To explore the postoperative psychological resilience of patients with thyroid cancer(TC)and its influencing factors.Methods One hundred and fifteen postoperative TC patients in the First Affiliated Hospital of Zhengzhou University from May 2019 to July 2022 were selected as the study population,and their psychological status was assessed using the Connor-Davidson resilience scale(CD-RISC),and factors influencing the postoperative CD-RISC scores of TC patients were analyzed by multiple linear regression.Results The postoperative CD-RISC score of 115 TC patients was(57.67±4.67)points,which was lower than the domestic norm[(65.40±13.90)points],with statistically significant differences(P<0.05).Comparison of CD-RISC scores among patients with different levels of education,tumor stage,general self-efficacy scale(GSES)scores,family functioning,and the five factor inventory-neuroticism(FFI-N)scores,and the difference was statistically significant(P<0.05).Multiple linear regression analysis showed that education level,tumor stage,GSES score,family function,and FFI-N score were factors influencing CD-RISC score in TC patients,with statistically significant differences(P<0.05).Conclusion The postoperative psychological resilience level of TC patients still needs to be improved,and its main influencing factors are education level,tumor stage,GSES score,family function,and FFI-N score.It is suggested that health care workers should dynamically assess their psychological resilience level and implement targeted interventions to improve their psychological status,maintain their physical and mental health,and improve their quality of life.
