摘要
目的 分析老年高血压患者赋能水平、执行能力对其健康素养的交互作用。方法 选取2021年4月至2023年1月河南省中西医结合医院收治的367例老年高血压患者,分别采用简化版老年人健康赋能量表(EHES)、成年人健康自我管理能力测评量表(AHSMSRS)、慢性病患者健康素养量表(HeLMS)对患者赋能水平、执行能力、健康素养进行调查,采用Pearson分析赋能水平、执行能力与健康素养关系,比较不同临床特征患者健康素养,采用多元线性回归分析健康素养的相关影响因素,使用交互作用系数γ和比值比(OR)分析赋能水平、执行能力对健康素养交互作用是否存在及其作用类型。结果 367例老年高血压患者EHES总分为(27.57±8.38)分;AHSMSRS总分为(141.12±16.78)分,标准化得分为73.44分;HeLMS总分为(85.58±13.94)分,标准化得分为73.01分。Pearson分析显示,赋能水平、执行能力与健康素养呈正相关(r_(1)=0.783,r_(2)=0.730,P<0.05)。多元线性回归分析显示,年龄、共同居住家庭人数>2人、大专及以上受教育程度、高血压分级3级、赋能水平、病程、执行能力与患者健康素养相关(P<0.05)。γ=1.153>1,赋能水平对执行能力的效应具有正向交互作用。结论 老年高血压患者赋能水平及执行能力处于中等水平,但其健康素养水平较低,受高血压分级、病程、受教育程度等因素影响,且赋能水平、执行能力对患者健康素养存在交互作用,临床可参照本研究分析制定针对性干预方案,以提高老年高血压患者健康素养。
Objective To analyze the interaction of empowerment level and executive ability of elderly hypertension cases on their health literacy.Methods A total of 367 elderly hypertensive patients admitted to Henan Integrative Medicine Hospital from April 2021 to January 2023 were selected to investigate the level of empowerment,executive ability and health literacy using the simplified version of the elderly health empowerment scale(EHES),the rating scale of health self-management skill for adults(AHSMSRS)and the health literacy scale for patients with chronic diseases(HeLMS),respectively.Pearson analysis was used to explore the relationship between empowerment level,executive ability and health literacy,health literacy in patients with different clinical characteristics were compared,multiple linear regression analysis was used to explore the relevant influences on health literacy,and analysis of the existence of the interaction effect of empowerment level,executive ability on health literacy and its type of effect using the interaction coefficientγand the odds ratio(OR).Results The total EHES score of 367 elderly hypertensive patients was(27.57±8.38),the total AHSMSRS score was(141.12±16.78),with a standardized score of 73.44,and the total HeLMS score was(85.58±13.94),with a standardized score of 73.01.Pearson analysis showed that the level of empowerment,executive ability and health literacy were positively correlated(r_(1)=0.783,r_(2)=0.730,P<0.05).Multiple linear regression analysis showed that age,number of family members living together>2,educational level college and above,hypertension classification level 3,empowerment level,disease duration and executive ability were associated with patients’health literacy(P<0.05).γ=1.153>1,and the effect of empowerment level on executive ability had a positive interaction.Conclusion Elderly patients with hypertension have an intermediate level of empowerment and executive ability,but their health literacy level is low,which is influenced by factors such as hypertension classification,disease duration and education level,and there is an interactive effect of empowerment level and executive ability on patients’health literacy,so the clinical can make reference to the analysis of this study to develop targeted intervention programs to improve the health literacy of elderly patients with hypertension.
