三维血管成像在肝脏血管瘤患者介入治疗中的应用

Application of Three-Dimensional Angiography in Interventional Treatment of Patients with Hepatic Hemangioma

目的分析CT三维血管成像在肝脏血管瘤患者介入治疗中的应用。方法选择2020年1月至2021年12月在永煤集团总医院收治的90例肝脏血管瘤患者作为研究对象,将患者随机分为两组,对照组(45例)接受动脉介入栓塞治疗,研究组(45例)在接受对照组栓塞治疗前应用CT三维血管成像,评估两组的临床疗效、定位和入路设计准确性及肝功能,并记录两组的并发症及复发情况。结果研究组的定位和入路设计准确性(77.78%、82.22%)高于对照组(57.78%、55.56%)(P<0.05)。研究组的血清丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、总胆红素(TBIL)水平低于对照组(P<0.05)。研究组的总有效率(91.11%)高于对照组(73.33%)(P<0.05),复发率(11.11%)低于对照组(28.89%)(P<0.05),并发症发生率(22.22%)稍低于对照组(26.67%),但差异无统计学意义(P>0.05)。结论平阳霉素、碘化油乳剂联合CT三维血管成像可有效提高肝脏血管瘤患者介入治疗的临床效果,并改善肝功能,且术中定位及手术入路设计的准确性较高,预后良好,可为临床肝脏血管瘤的介入治疗提供参考依据。

Objective To analyze the application of CT three-dimensional angiography in interventional therapy of patients with hepatic hemangioma.Methods A total of 90 patients with liver hemangioma admitted to Yongmei Group General Hospital from January 2020 to December 2021 were selected as the research objects.The patients were randomly divided into two groups,the control group(45 cases)was received arterial interventional embolization,while the study group(45 cases)was treated with CT three-dimensional angiography before the treatment of the control group.The clinical efficacy,accuracy of localization and approach design and liver function were evaluated.Complications and recurrence were recorded in the two groups.Results The accuracy of localization and approach design in the study group(77.78%,82.22%)was higher than that in the control group(57.78%,55.56%)(P<0.05).The serum levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST)and total bilirubin(TBIL)in the study group were lower than those in the control group(P<0.05).The total effective rate of the study group(91.11%)was higher than that of the control group(73.33%)(P<0.05),the recurrence rate(11.11%)was lower than that of the control group(28.89%)(P<0.05),and the complication rate(22.22%)was slightly lower than that of the control group(26.67%),but the difference was not statistically significant(P>0.05).Conclusion Pingyangmycin,iodized oil emulsion combined with CT three-dimensional angiography can effectively improve the clinical efficacy of interventional therapy for patients with hepatic hemangioma,and improve liver function,and the accuracy of intraoperative localization and surgical approach design is high,and the prognosis is good,which can provide a reference for clinical interventional therapy of hepatic hemangioma.
