精浆弹性蛋白酶与男性不育症精子DNA完整性和精子质量的相关性分析

Correlation analysis between seminal plasma elastase and sperm DNA integrity and sperm quality inmale infertility

目的分析精浆弹性蛋白酶与男性不育症患者精子DNA完整性和精子质量的相关性,为临床应用提供依据。方法选取2020年1月至2022年1月北京家恩德运医院收治的180例男性不育症患者作为研究对象,设为观察组。另选取同期的100例正常男性作为对照组。检测受试者精浆弹性蛋白酶、精子DNA碎片率(DFI)、a级和b级精子百分率、精子密度及活力水平;采用Spearman相关性检验分析精浆弹性蛋白酶与精子DNA完整性和精子质量的相关性。结果观察组精浆弹性蛋白酶、DFI、精子密度均高于对照组,a级或b级精子百分率、精子活力均明显低于对照组,差异具有统计学意义(P<0.05)。精浆弹性蛋白酶高表达组精浆弹性蛋白酶、DFI、精子密度均明显高于正常表达组,a级精子百分率、b级精子百分率和精子活力均明显低于正常表达组,差异具有统计学意义(P<0.05)。男性不育症患者精子DFI、精子密度与精浆弹性蛋白酶呈显著正相关,与a级或b级精子百分率、精子活力、精浆弹性蛋白酶呈显著负相关(P<0.05)。结论男性不育症患者精子DNA完整性、精子密度与精浆弹性蛋白酶、精子质量呈高度相关关系。

Objective To analyze the correlation between seminal plasma elastase and sperm DNA integrity and sperm quality in male infertility,to provide a basis for clinical application.Methods A total of 180 male infertility patients admited to Beijing Jjia'en Hospital from January 2020 to January 2022 were selected as the research subjects and set as the observation group.100 nomal males were selected as the control group.The subjects'seminal plasma elastase,sperm DNA fragmentation index(DFT),percentage of grade a and b spem,sperm density and motility level were detected.Spearman correlation test was used to analyze the correlation between seminal plasma elastase and sperm DNA integrity and sperm quality.Results The seminal plasma elastase,DFI and sperm density of the observation group were higher than those of the control group,and the percentage of grade a or b spem and spem motility were significantly lower than those of the control group,and the diference was statistically significant(P<0.05).The seminal plasma elastase,DFI and sperm density of the patients with high protease expression group were significantly higher than those of the normal expression group,and the percentage of grade a sperm,grade b spem percentage and sperm motility were significantly lower than those of the normal expression group,and the difference was statistic ally significant(P<0.05).Sperm DFI and sperm density in male infertile patients were significantly positively correlated with seminal plasma elastase,and the percentage of grade a or b sperm and sperm motility were significantly negatively correlated with seminal plasma elastase,with a statistical significance(P<0.05).Conclusions Sperm DNA integrity and sperm density were highly correlated with seminal plasma elastase and spem quality in male infertile patients.
