视听性刺激联合他达拉非诊断心理性勃起功能障碍的临床研究

Clinical study of audiovisual sexual stimulation combined with tadalafil in the diagnosis of psychologicalerectile dysfunction

目的探讨视听性刺激(AVSS)联合他达拉非诊断心理性勃起功能障碍(ED)的临床价值。方法选取2019年9月至2021年8月聊城市人民医院男科诊治的551例ED患者作为研究对象,均进行勃起功能检测。根据国际勃起功能指数-5(IIEF-5)评分分为轻度组、中度组、重度组,计算三组正常勃起、有效勃起及正常勃起占有效勃起的比例,统计三组勃起硬度≥60%持续时间、冠状沟平均硬度、冠状沟平均胀大周径、基底部平均硬度及基底部平均胀大周径,并进行组间比较;根据检测结果分成心理性ED组和非心理性ED组,比较分析两组的ED相关危险因素。结果轻、中、重度组正常勃起、有效勃起及正常勃起占有效勃起的比例随ED程度加重而下降,三组勃起参数比较,轻度组优于中度组,中度组优于重度组,差异具有统计学意义(P<0.05);心理性ED组和非心理性ED组年龄≥40岁、吸烟史>5年、饮酒史>10年、高血压、糖尿病、夫妻关系不和、心理负荷过重等占比比较,差异具有统计学意义(P<0.05)。结论AVSS联合他达拉非诊断心理性ED与IIEF-5评分显示出有较高的匹配度,且具备良好的客观性。

Objective To investigate the clinical value of audiovisual sexual stimulation(AVSS)combined with tadalafl in the diagnosis of psychological erectile dyfunction(ED).Methods A total of 551 ED patients diagnosed and treated in Department of Andrology of liaocheng People's Hospital were selected as research objects from September 2019 to August 2021.All of them were tested for ED.They were divided into mild group,moderate group and severe group according to international index of erectile function-5(IIEF-5)score,and the proportion of normal erection,effective erection and normal erection to effective erection in the three groups was calculated.The duration of erectile hardness≥60%,the avenage hardness and dilated circumference of coronary ditch,the average hardness and dilated circumnference of base were recorded and compared among the three groups.According to the test results,patients were divided into psychological ED group and non-psychological ED group,and the ED related risk factors was compared between the two groups.Results In the rmild,moderate and severe groups,the proportion of normal erection,ratio of effective erection and normal erection to effective erection decreased with the aggravation of ED.The comparison of erectile parameters of each group showed that the rmild group was better than the moderate group,and the moderate group was better than the severe group,the difference was statistically sigificant(P<0.05);Men with age≥40 years old,smoking history>5 years,drinking history>10 years,hypertension,diabetes,marital discord,and psychological overload were compared between the psychological ED group and the non-psychological ED group,the differences were stistically signifcant(P<0.05).Conclusions AVSS combined with tadalafil showed a high match with IEF-5 score in the diagnosis of psychological ED with good objectivity.
