肾素-血管紧张素-醛固酮系统基因多态性与先兆子痫相关性的Meta分析

Association between renin-angiotensin-aldosterone system gene polymorphisms and preeclampsia: aMeta-analysis

目的评价肾素-血管紧张素-醛固酮系统中的4个重要基因多态性与先兆子痫的相关性。方法检索Pubmed、Web of Science、Science Direct、NCBI、万方和维普数据库中肾素-血管紧张素-醛固酮系统基因多态性与先兆子痫相关性的文献,检索时间为建库至2022年10月18日,纳入4个基因多态性与先兆子痫相关性的病例对照研究,使用R 4.2.1进行Meta合并效应量分析,采用漏斗图和Egger′s检验评估发表偏倚。结果Meta分析结果发现,CYP11B2-C344T多态性显性模型下与先兆子痫的发生有相关性(RR=0.78,95%CI:0.62~0.97,P=0.01);ACE(I/D)基因多态性显性模型下与先兆子痫的发生有相关性(RR=0.95,95%CI:0.89~1.01,P<0.01);AGTR1-A1166C基因多态性显性模型下与先兆子痫的发生无相关性(RR=1.0,95%CI:0.99~1.02,P=0.40);AGT-M235T基因多态性显性模型下与先兆子痫的发生有相关性(RR=1.01,95%CI:0.95~1.08,P<0.01)。结论CYP11B2-C344T、ACE(I/D)和AGT-M235T基因多态性可能和先兆子痫的发生具有相关性,携带风险基因的人群需要注意预防先兆子痫。

Objective To evaluate the relationship between the polymorphisms of 4 impotant genes in renin-angiotensin-aldosterone system(RAAS)and preeclampsia.Methods PubMed,Web of Science,ScienceDirect,NCBI,Wanfang and VIP databases were used to search for literature related to RAAS gene polymorphisms and pre-eclampsia,the retrieval time was from the establishment of the database to October 18,2022.A number of case-control study of the gene polymophisms of 4 important genes associated with preeclampsia were included.Meta-analysis combined effect size analysis was performed using R 4.2.1,and publication bias was assessed using funnel plots test and Egger's test.Results The results of Meta analysis found that CYP11 B2-C344T polymorphism was associated with the occurrence of pre-eclampsia under the doninant model(RR=0.78,95%Cl;0.62-0.97,P=0.01),ACE(I/D)polymorphism was associated with the occurrence of pre eclampsia under the dominant model(RR=0.95,95%CI:0.89-1.01,P<0.01),AGTR1-A1166C polymorphism was not associated with the occurence of pre eclampsia in the dominant model(RR=1.0,95%CI:0.99-1.02,P=0.40);AGT-M235T polymophism was associated with the occurrence of pre-eclampsia under the dominant model(RR=1.01,95%CI;0.95-1.08,P<0.01).Conclusions CYP11B2-C344T,ACE(I/D)and AGT-M235T polymophisms may be associated with the occurrence of preeclampsia.This suggests that people with risk polymorphisms need to pay attention to preventing pre eclampsia.
