中西医结合治疗多发性抽动症38例临床观察

Clinical Observation on 38 Cases of Tourette Syndrome Treated with Integrated Traditional Chinese and Western Medicine

目的:探究中西医结合治疗治疗多发性抽动症(TS)患儿的临床疗效。方法:选取我院76例TS患儿作为研究对象,按照随机数字表法分为对照组(n=38)和观察组(n=38),对照组采用行为干预治疗;观察组在对照组基础上加用调肝熄风针法治疗。比较两组疗效、治疗前后耶鲁综合抽动严重程度量表(YGTSS)评分、神经功能指标[5-羟色胺(5-HT)、多巴胺(DA)、γ-氨基丁酸(GABA)]、血清促肾上腺皮质激素(ACTH)、皮质醇(COR)水平。结果:治疗后观察组YGTSS评分中各维度及总分低于对照组(P<0.05);治疗后观察组血清5-HT、DA水平低于对照组,GABA水平高于对照组(P<0.05);治疗后观察组血清ACTH、COR水平低于对照组(P<0.05);观察组总有效率94.74%高于对照组73.68%(P<0.05)。结论:采用中西医结合,即调肝熄风针法联合行为干预治疗TS患儿效果显著,可通过下调下丘脑-垂体-肾上腺(HPA)轴功能调节神经递质水平,改善临床症状。

Objective To explore the clinical effect of Integrated Chinese and Western Medicine on children with Tourette syndrome(TS).Methods 76 children with TS in our hospital were selected as the research objects.They were randomly divided into the control group(n=38)and the observation group(n=38).The control group was treated with behavioral intervention,and the observation group was treated with Tiaogan Xifeng acupuncture on the basis of the control group.The curative effects of the two groups were compared.The Yale comprehensive tic severity scale(YGTSS)score and neurological indicators included 5-hydroxytryptamine(5-HT),dopamine(DA),γ-Aminobutyric acid(GABA),serum adrenocorticotropic hormone(ACTH)and cortisol(COR)levels.Results After treatment,the YGTSS scores in the observation group were lower than those in the control group(P<0.05);After treatment,the levels of serum 5-HT and DA in the observation group were lower than those in the control group,and the levels of GABA were higher than those in the control group(P<0.05);After treatment,the levels of serum ACTH and cor in the observation group were lower than those in the control group(P<0.05);The total effective rate in the observation group was 94.74%higher than 73.68%in the control group(P<0.05).Conclusion Tiaogan Xifeng acupuncture acupuncture combined with behavioral intervention is effective in the treatment of TS children.It can regulate the level of neurotransmitters and improve clinical symptoms by down regulating the function of hypothalamus pituitary adrenal(HPA)axis.
