祛毒生肌汤内服联合大黄芒硝外敷用于妇科腹部手术切口愈合障碍的临床价值

Clinical Value of Qudu Shengji Decoction(祛毒生肌汤) Combined with External Application of Dahuang(Rhubarb) and Mangxiao(Mirabilite) for Incision Healing Disorder in Gynecological Abdominal Surgery

目的 探讨祛毒生肌汤内服联合大黄芒硝外敷在妇科腹部手术切口愈合障碍中的应用价值。方法 选择医院妇科2020年1月—2021年5月收治的80例腹部手术切口愈合障碍患者进行研究,使用随机数字表通过简单随机法将其分为研究组与对照组每组40例,两组在接受常规伤口处理的基础上,给予对照组大黄芒硝外敷干预,研究组在此基础上,联合使用祛毒生肌汤内服治疗。比较两组患者治疗前后炎症指标[C反应蛋白(C reactive protein, CRP)、肿瘤坏死因子α(tumor necrosis factor α,TNF-α)]水平、切口愈合效果、硬结与表面红消退时间、并发症以及满意度情况。结果 研究组治疗后CRP、TNF-α水平均明显低于对照组(P<0.05),研究组总有效率明显高于对照组,研究组总满意度为95.00%(38/40),高于对照组80.00%(32/40),组间比较差异具有统计学意义(P<0.05),研究组硬结消退时间与表面红消退时间均短于对照组(P<0.05),研究组并发症发生率为7.50%(3/40),低于对照组(25.00%,10/40)(P<0.05)。结论 祛毒生肌汤内服联合大黄芒硝外敷能够明显改善妇科腹部手术切口愈合情况,提高患者满意度,适合推广应用。

Objective To investigate the application value of Qudu Shengji Decoction(祛毒生肌汤)for oral administration combined with external application of Dahuang(Rhubarb)and Mangxiao(Mirabilite)in the treatment of gynecological abdominal surgery incision healing disorders.Methods A total of 80 patients with abdominal surgery incision healing disorders who were admitted to the gynecology department of the hospital from January 2020 to May 2021 were selected for research,and they were divided into a study group and a control group by a simple random method using a random number table,40 cases in each group.On the basis of routine wound treatment,the control group was given external application of Dahuang(Rhubarb)and Mangxiao(Mirabilite)to intervene,and the study group was given Qudu Shengji Decoction for oral treatment on this basis.The levels of inflammatory indexes[C reactive protein(CRP),tumor necrosis factorα(TNF-α)],incision healing effect,induration and superficial red resolution time,complications and satisfaction were compared between the two groups before and after treatment.Results After treatment,the levels of CRP and TNF-αin the study group were significantly lower than those in the control group(P<0.05),and the total effective rate in the study group was significantly higher than that in the control group.The difference between the groups was statistically significant(P<0.05).The induration and superficial redness in the study group were shorter than those in the control group(P<0.05).The incidence of complications in the study group was 7.50%(3/40),which was lower than that in the control group(25.00%,10/40)(P<0.05).Conclusion The oral administration of Qudu Shengji Decoction combined with external application of Dahuang(Rhubarb)and Mangxiao(Mirabilite)can significantly improve the healing of gynecological abdominal surgery incisions and improve patients satisfaction,suitable for popularization and application.
