富白细胞-血小板纤维蛋白联合聚己双胍复合液治疗Wagner3级糖尿病足创面的效果

摘要 目的探讨富白细胞-血小板纤维蛋白(L-PRF)联合聚己双胍复合液治疗Wagner3级糖尿病足创面的效果。方法选取2021年7月至2022年10月南昌大学第四附属医院内分泌科收治的60例Wagner3级糖尿病足患者作为研究对象,按照随机数字表法分为对照组(30例)与观察组(30例)。对照组采用L-PRF治疗,观察组采用L-PRF联合聚己双胍复合液治疗。比较两组患者的溃疡面积、创面愈合率、创面细菌培养阳性率、创面治疗总有效率、患者住院时间及住院费用。结果两组患者治疗前的溃疡面积比较,差异无统计学意义(P>0.05);两组患者治疗后的溃疡面积均小于治疗前,差异有统计学意义(P<0.05);观察组患者治疗后的溃疡面积小于对照组,创面愈合率高于对照组,差异有统计学意义(P<0.05)。两组患者治疗后的创面细菌培养阳性率均低于治疗前,观察组患者治疗后的创面细菌培养阳性率低于对照组,差异有统计学意义(P<0.05)。观察组患者的治疗总有效率高于对照组,差异有统计学意义(P<0.05)。观察组患者的住院时间短于对照组,住院费用少于对照组,差异有统计学意义(P<0.05)。结论L-PRF联合聚己双胍复合液治疗Wagner3级糖尿病足创面,能有效促进创面愈合,提高细菌清除率和治疗效果,缩短住院时间,降低治疗成本,值得临床推广应用。 Objective To explore the effect of leucocyte-platelet rich fibrin(L-PRF)combined with Polyhexanide Complex Solution in the treatment of Wagner grade 3 diabetes foot wounds.Methods A total of 60 patients with Wagner grade 3 diabetes foot admitted to the Department of Endocrinology in the Fourth Affiliated Hospital of Nanchang University were selected as the research objects from July 2021 to October 2022,they were divided into the control group(30 cases)and the observation group(30 cases)according to the random number table.The control group was treated with L-PRF,while the observation group was treated with L-PRF combined with Polyhexanide Complex Solution.The ulcer area,wound healing rate,positive rate of wound bacterial culture,total effective rate of wound treatment,hospitalization time,and hospitalization cost of the two groups of patients were compared.Results There was no statistically significant difference in ulcer area between the two groups of patients before treatment(P>0.05).The ulcer area after treatment in both groups of patients was smaller than before treatment,the difference was statistically significant(P<0.05).The ulcer area of the observation group patients after treatment was smaller than that of the control group,and the wound healing rate was higher than that of the control group,the differences were statistically significant(P<0.05).The positive rate of wound bacterial culture in both groups of patients after treatment was lower than before treatment,while the positive rate of wound bacterial culture in the observation group was lower than that in the control group,the differences were statistically significant(P<0.05).The total effective rate of treatment in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).The hospitalization time of the observation group patients was shorter than that of the control group,and the hospitalization cost was less than that of the control group,the differences were statistically significant(P<0.05).Conclusion L-PRF combined with Polyhexanide Complex Solution can effectively promote wound healing,improve bacterial clearance rate and treatment effect,shorten hospital stay,and reduce treatment costs in the treatment of Wagner Grade 3 diabetes foot wounds,which is worthy of clinical application.
