摘要
目的:分析2型糖尿病(type 2 diabetes mellitus,T2DM)患者血清糖类抗原19-9(carbohydrate antigen 19-9,CA19-9)、糖类抗原125(carbohydrate antigen 125,CA125)、癌胚抗原(carcinoembryonic antigen,CEA)和甲胎蛋白(alpha fetoprotein,AFP)的表达水平及与糖脂代谢的相关性。方法:选择2021年1月至2022年6月重庆医药高等专科学校附属第一医院内分泌代谢科收治的296例T2DM患者作为观察组,随机选取同期在本院进行健康体检的300例体检者作为对照组,比较两组血清肿瘤标志物水平,进一步将T2DM患者按照肿瘤标志物水平分为升高组和正常组,对比分析两亚组糖代谢、脂代谢相关指标的差异;采用Pearson相关性分析肿瘤标志物与糖脂代谢指标的相关性。结果:T2DM患者的血清CA19-9、CA125和CEA水平高于健康对照人群,差异有统计学意义(均P <0.05)。肿瘤标志物升高组的空腹血糖(fasting plasma glucose,FPG)、餐后2小时血糖(2-hour postprandial plasma gloucose,2h PG)、糖化血红蛋白(glycosylated hemoglobin A1C,HbA1C)、总胆固醇(total cholesteral,TC)、甘油三酯(triglyceride,TG)明显高于肿瘤标志物正常组,而高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)则低于肿瘤标志物正常组,差异均有统计学意义(均P <0.05)。T2DM患者血清CA19-9与FPG、2h PG、HbA1C、TC、TG、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)呈正相关关系,CA125与HbA1C呈正相关关系,CEA与FPG、2h PG、HbA1C、LDL-C呈正相关关系,差异均有统计学意义(均P <0.05)。结论:T2DM患者存在CA19-9、CA125和CEA等部分肿瘤标志物含量增高,与糖脂代谢异常相关。
Objective:To analyze the expression levels of serum carbohydrate antigen 19-9(CA19-9),carbohydrate antigen 125(CA125),carcinoembryonic antigen(CEA)and alpha fetoprotein(AFP)in patients with type 2 diabetes mellitus(T2DM)and their correlation with glucose and lipid metabolism.Methods:296 T2DM patients admitted to the department of endocrinology and metabolism in the First Affiliated Hospital of Chongqing Medical and Pharmaceutical College from January 2021 to June 2022 were selected as the observation group,and 300 physical examinees in our hospital in the same period were randomly selected as the control group.The levels of serum tumor markers in the two groups were compared.T2DM patients were further assigned to the elevated group and the normal group according to the levels of tumor markers,and differences in glucose and lipid metabolism-related indicators between the two groups were compared.The correlation between tumor markers and glucose and lipid metabolism was analyzed by Pearson correlation.Results:The levels of serum CA19-9,CA125 and CEA in T2DM patients were significantly higher than those in healthy controls(all P<0.05).Fasting plasma glucose(FPG),2-hour postprandial plasma glucose(2h PG),glycosylated hemoglobin A1C(HbAlc),total cholesterol(TC)and triglyceride(TG)in the group with elevated tumor markers were significantly higher than those in the group with normal tumor markers,while high-density lipoprotein cholesterol(HDL-C)in the former was lower than that in the later,and the differences were statistically significant(all P<0.05).Serum CA19-9 in patients with T2DM was positively correlated to FPG,2h PG,HbAlc,TC,TG and low-density lipoprotein cholesterol(LDL-C);CA125 was positively correlated to HbAlc;CEA was positively correlated to FPG,2h PG,HbA1c and LDL-C;and the differences were statistically significant(all P<O.05).Conclusion:T2DM patients have increased levels of some tumor markers such as CA19-9,CA125 and CEA,which are related to abnormal glucose and lipid metabolism.
