经脐单孔腹腔镜手术治疗小儿阑尾炎的效果评价

EFFECT EVALUATION OF TRANSUMBILICAL SINGLE-PORT LAPAROSCOPIC SURGERY FOR PEDIATRIC APPENDICITISYU

目的 对比研究经脐单孔腹腔镜手术与传统三孔腹腔镜手术治疗小儿阑尾炎的疗效差异。方法 回顾性分析我院手术治疗的阑尾炎患儿264例,其中173例实施经脐单孔腹腔镜阑尾切除术;91例实施传统三孔腹腔镜阑尾切除术。比较二组手术时间、术中出血量、术后进食时间、住院时间、住院费用、切口美观程度、术后并发症情况差异。结果 二组手术时间、住院费用比较,差异无统计学意义(P>0.05);单孔组术中出血量、术后进食时间、住院时间均少(短)于对照组(P<0.05);单孔组术后腹壁无可见瘢痕,美容效果优于三孔组。结论 经脐单孔腹腔镜阑尾切除术治疗小儿阑尾炎不增加手术时间、住院费用和并发症,但能够减少术中出血,缩短术后进食时间、缩短住院时间,并且切口美容效果更好。

Objective To compare the curative effect of transumbilical single-port laparoscopic surgery and traditional three-port laparoscopic surgery in the treatment of pediatric appendicitis. Methods A total of two hundred and sixty-four children with appendicitis who underwent surgical treatment in our hospital were retrospectively analyzed.Among them, 173 underwent transumbilical single-port laparoscopicappendectomy.Conventional three-port laparoscopic appendectomy was performed in 91 cases.The operation time, intraoperative blood loss, postoperative feeding time, hospitalization time, hospitalization cost, incision aesthetics and postoperative complications were compared between the two groups. Results There was no significant difference in operation time and hospitalization cost between the two groups(P<0.05).The intraoperative blood loss, postoperative feeding time and hospital stay in the single-well group were less than those in the control group(P<0.05).There was no visible scar on abdominal wall in the single-hole group, and the cosmetic effect was better than that in the three-hole group. Conclusion Transumbilical single-port laparoscopic appendectomy in the treatment of pediatric appendicitis does not increase the operation time, hospitalization costs and complications, but can reduce intraoperative bleeding, shorten the postoperative feeding time, shorten the length of hospital stay, and the incision cosmetic effect is better.
