“活血化浊通窍利咽法”干预缺血性脑卒中吞咽障碍的临床评价研究

Clinical Evaluation on Intervention of Swallowing Disorders in Ischemic Stroke with Huoxue Huazhuo Tongqiao Liyan Method

目的:探究“活血化浊通窍利咽法”对缺血性脑卒中吞咽障碍的治疗作用。方法:将60例缺血性脑卒中吞咽障碍(浊瘀毒损型)受试者进行随机分组(观察组31例与对照组29例)。对照组给予基础治疗以及常规吞咽功能康复训练,观察组在对照组干预基础上,根据“活血化浊通窍利咽法”给予中药进行干预,干预周期均为3 w。通过比较2组受试者干预前后洼田饮水试验评分、标准吞咽功能评价量表(SSA)评分及中医证候综合积分,进行疗效评价及安全性评估。结果:观察组与对照组干预后的洼田饮水试验评分、SSA评分均较干预前降低(P<0.05),且观察组上述评分低于对照组(P<0.05)。2组受试者干预后的中医证候综合积分均较干预前降低(P<0.05),且观察组该积分低于对照组(P<0.05)。结论:活血化浊通窍利咽法对缺血性脑卒中吞咽障碍具有明显改善作用,能够降低受试者的中医证候评分,且该方法安全性较好,无不良反应发生。

Objective:To explore the therapeutic effect of the Huoxue Huazhuo Tongqiao Liyan method(Blood-activating turbid-eliminating orifice-opening swallow-promoting method)on swallowing disorders in ischemic stroke.Method:Sixty subjects with swallowing disorders(turbid-stasis damage type)after ischemic stroke were randomly divided into an observation group of 31 cases and a control group of 29 cases.The control group received basic treatment and routine swallowing function rehabilitation training.On the basis of intervention in the control group,the observation group received Chinese medicine treatment according to the Huoxue Huazhuo Tongqiao Liyan method,with a treatment period of 3 weeks.By comparing Kubota Water Swallowing Test(WST),Standard Swallowing Function Assessment Scale(SSA),and TCM syndrome comprehensive score before and after intervention between the two groups,efficacy and safety evaluations were conducted.Result:After intervention,the WST scores and SSA scores in both groups decreased compared with that before treatment(P<0.05),and the above scores of the observation group were lower than those of the control group(P<0.05).The comprehensive scores of TCM syndromes in both groups decreased after intervention(P<0.05),and the scores in the observation group were lower than those in the control group(P<0.05).Conclusion:The Huoxue Huazhuo Tongqiao Liyan method has a significant improvement effect on swallowing disorders in ischemic stroke,which can reduce the TCM syndrome score of the subjects.Moreover,this method is safe without adverse reactions.
