CO_(2)激光联合光动力治疗尿道口、肛周尖锐湿疣的临床效果

Clinical efficacy of CO_(2)laser combined with photodynamic therapy in the treatment of urethral orifice and perianal condyloma acuminatum

目的:对比CO_(2)激光联合光动力与单纯CO_(2)激光治疗尿道口、肛周尖锐湿疣的临床疗效。方法:回顾性分析2020年6月至2022年8月中国人民解放军联勤保障部队第904医院收治的尿道口、肛周尖锐湿疣患者102例,行单纯CO_(2)激光治疗者为对照组(51例),行CO_(2)激光联合光动力治疗者为试验组(51例)。比较2组临床疗效和治疗前后血清增殖细胞核抗原(proliferating cell nuclear antigen,PCNA)、血管内皮细胞生长因子(vascular endothelial growth factor,VEGF)水平、T淋巴细胞亚群水平,并比较2组不良反应发生率、复发率。结果:试验组临床总有效率较对照组高(P<0.05);试验组治疗后血清PCNA、VEGF水平均较对照组低(均P<0.05);试验组治疗后T淋巴细胞亚群CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均较对照组高(均P<0.05);试验组治疗后3个月疾病复发率较对照组低(P<0.05)。结论:CO_(2)激光联合光动力治疗尿道口、肛周尖锐湿疣疗效明显优于单纯CO_(2)激光治疗,并可降低血清PCNA、VEGF水平,调节T淋巴细胞亚群水平,值得推广。

Objective:To compare the clinical efficacy of CO_(2)laser combined with photodynamic therapy and CO_(2)laser alone in the treatment of urethral orifice and perianal condyloma acuminatum.Methods:A retrospective study was used to select 102 patients with urethral orifice and perianal condyloma acuminatum admitted to 904 Hospital of the Joint Service Support Force of the People’s Liberation Army from June 2020 to August 2022.The patients who were treated with CO_(2)laser alone as the control group(51 cases),and the patients who were treated with CO_(2)laser combined with photodynamic therapy as the test group(51 cases).The clinical efficacy,the levels of serum proliferating cell nuclear antigen(PCNA),vascular endothelial growth factor(VEGF),T lymphocyte subsets,the incidence of adverse reactions and the recurrence rate were compared between the 2 groups before and after treatment.Results:The total clinical effective rate of the test group was higher than that of the control group(P<0.05).The levels of serum PCNA and VEGF in the experimental group were lower than those in the control group(both P<0.05).After treatment,the levels of CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in the test group were higher than those in the control group(all P<0.05).The disease recurrence rate in the test group was lower than that in the control group 3 months after treatment(P<0.05).Conclusion:The curative effect of CO_(2)laser combined with photodynamic therapy for urethral orifice and perianal condyloma acuminatum is significantly better than that of CO_(2)laser alone.It can reduce the level of serum PCNA and VEGF,regulate the level of T lymphocyte subsets,which is worthy of popularization.
