摘要
目的探讨T2DM患者轻度认知功能障碍(MCI)与血糖在目标范围内时间(TIR)、低于目标范围时间(TBR,1级低血糖、2级低血糖)和高于目标范围时间(TAR)的相关性。方法选取2020年1月至2020年12月于山东大学附属威海市立医院住院治疗的T2DM患者158例,根据蒙特利尔认知评估量表(MoCA)评分分为MCI组(n=83)和T2DM组(n=75),记录TIR、TBR(1级低血糖、2级低血糖)、TAR。Spearman相关分析MoCA评分与各指标的相关性,Logistic回归分析MCI的影响因素。结果与T2DM组比较,MCI组TBR、TAR升高(P<0.01),TIR降低(P<0.01)。Spearman相关分析显示,MoCA评分与TIR呈正相关(P<0.01),与DM病程、HbA1c、TBR、TAR呈负相关(P<0.05),其中2级低血糖相关性强于1级低血糖(P<0.01)。Logistic回归分析显示,HbA1c、TIR、TBR、TAR、2级低血糖是MCI的影响因素。结论HbA1c、TBR、TAR、2级低血糖是T2DM患者MCI的危险因素。
Objective To explore the correlation between mild cognitive impairment(MCI)and time in target glucose range(TIR),time below target glucose range(TBR,grade 1 hypoglycemia,grade 2 hypoglycemia)and time above target glucose range(TAR)of blood glucose in patients with type 2 diabetes mellitus(T2DM).Methods 158 T2DM patients hospitalized in Weihai Municipal Hospital Affiliated with Shandong University from January 2020 to December 2020 were selected and divided into MCI group(n=83)and T2DM group(n=75)based on the score of Montreal Cognitive Assessment Scale(MoCA).TIR,TBR(grade 1 hypoglycemia,grade 2 hypoglycemia)and TAR were recorded.Spearman correlation analysis was used to analyze the correlation between MoCA score and various indicators.Logistic regression analysis was used to analyze the influencing factors of cognitive impairment.ResultsCompared with T2DM group,the proportions of TBR and TAR in MCI group increased(P<O.O1),while the proportion of TIR decreased(P<0.01).Spearman correlation analysis showed that MoCA score was positively correlated with TIR(P<0.01),and negatively correlated with DM duration,HbAic,TBR,and TAR(P<0.05).The correlation between grade 2 hypoglycemia and cognitive impairment is stronger than that of grade 1 hypoglycemia(P<0.O1).Logistic regression analysis shows that HbAic,TIR,TBR,TAR,and grade 2 hypoglycemia were influencing factors for MCI.Conclusion HbAic,TBR,TAR and grade 2 hypoglycemia may be the risk factors for MCI in T2DM.
