情志干预联合中药熏蒸辅助甲钴胺治疗肱骨干骨折术后桡神经损伤的效果

Effect of Emotional Intervention Combined with Traditional Chinese Medicine Fumigation Assisted with Mecobalamin on Radial Nerve Injury After Humeral Shaft Fracture Surgery

目的:研究情志干预联合中药熏蒸在肱骨干骨折术后桡神经损伤患者中的应用效果.方法:选取濮阳市安阳地区医院2020年8月—2022年8月收治的肱骨干骨折术后桡神经损伤患者116例作为研究对象,根据随机、开放、对照试验原则,采用随机信封法分为对照组、观察组,每组58例.对照组给予常规干预,观察组在此基础上加用情志干预联合中药熏蒸,均干预4周.比较两组干预前后情绪状态[Zung焦虑、抑郁自评量表(SAS、SDS)]、上肢运动功能[Fugl-Meyer运动功能测评法(FMA)]、疼痛程度[数字评价量表(NRS)]、桡神经传导速度,并对比两组康复依从性.结果:观察组SAS、SDS、NRS评分低于对照组(P<0.05);观察组康复依从性为98.28%,高于对照组的84.48%(P<0.05);观察组上肢FMA评分、MCV、SCV高于对照组(P<0.05).结论:情志干预联合中药熏蒸能降低肱骨干骨折术后桡神经损伤患者疼痛程度,改善消极情绪,提高康复依从性,从而促进桡神经功能与上肢运动功能恢复.

Objective:To study the application effect of emotional nursing intervention combined with traditional Chinese medicine fumigation in patients with radial nerve injury after humeral shaft fracture.Methods:A total of 116 patients with radial nerve injury after humeral shaft fracture surgery admitted to Anyang District Hospital of Puyang City from August 2020 to August 2022 were selected as the research objects.According to the principle of random,open and controlled trial,they were divided into control group and observation group by random envelope method,with 58 patients in each group.The control group was given routine nursing intervention,and the observation group was given emotional nursing intervention combined with traditional Chinese medicine fumigation on this basis for 4 weeks.The emotional state[Zung anxiety,self-rating depression scale(SAS,SDS)],upper limb motor function[Fugl-Meyer motor function assessment method(FMA)],pain degree[numerical rating scale(NRS)]and radial nerve conduction velocity were compared between the two groups before and after intervention,and the rehabilitation compliance of the two groups was compared.Results:The scores of SAS,SDS and NRS in the observation group were lower than those in the control group(P<0.05).The rehabilitation compliance of the observation group was 98.28%,which was higher than 84.48%of the control group(P<0.05).The upper limb FMA score,MCV and SCV in the observation group were higher than those in the control group(P<0.05).Conclusion:Emotional nursing combined with traditional Chinese medicine fumigation can reduce the pain degree of patients with radial nerve injury after humeral shaft fracture surgery,improve negative mood,improve rehabilitation compliance,and promote the recovery of radial nerve function and upper limb motor function.
