2型糖尿病患者腰椎骨髓水含量磁共振T2 mapping定量分析

Quantitative analysis on water content of lumbar bone marrow in patients with type 2 diabetes mellitus by T2 mapping magnetic resonance

目的采用磁共振T2 mapping序列定量评价2型糖尿病(T2DM)患者腰椎骨髓水含量,并与非糖尿病患者进行比较,分析其与年龄、性别及体重指数(BMI)的关系。方法前瞻性分析2021年7~11月行腰椎MRI检查患者105例,扫描过程中采用T2 mapping脉冲序列进行扫描。通过后处理工作站,在T2 mapping像上测量腰1~5椎体的T2 mapping值。比较T2DM组和非糖尿病组腰椎骨髓平均T2 mapping值有无差异。比较L1~L5各椎体T2DM组和非糖尿病组骨髓T2 mapping值有无差异。比较T2DM患者女性和男性、BMI正常组和超重组腰椎骨髓平均T2 mapping值有无差异。分析T2DM患者、非糖尿病患者腰椎骨髓平均T2mapping值与年龄的相关性。比较女性和男性年龄≤50岁组与>50岁组腰椎骨髓平均T2 mapping值有无差异。结果T2DM组腰椎骨髓平均T2 mapping值低于非糖尿病患者(P<0.05)。L1~L4各椎体T2DM组骨髓T2 mapping值低于非糖尿病患者(P<0.05)。T2DM患者女性和男性、全部患者BMI正常组和超重组腰椎骨髓平均T2值差异无统计学意义(P>0.05)。T2DM组、非糖尿病组腰椎骨髓平均T2mapping值与年龄均呈负相关(P<0.05)。女性年龄≤50岁组腰椎骨髓平均T2 mapping值高于>50岁组(P<0.05)。结论T2DM影响腰椎骨髓水含量,T2DM患者腰椎骨髓水含量与年龄有关,与BMI无明显关系;女性激素水平可能是影响腰椎骨髓水含量的一个重要因素。

Objective To quantitatively evaluate the water content of lumbar bone marrow in patients with type 2 diabetes mellitus(T2DM)versus non-diabetic patients by T2 mapping magnetic resonance,and to analyze its correlation with age,gender and body mass index(BMI).Methods A total of 105 patients who underwent lumbar magnetic resonance imaging(MRI)from July 2021 to November 2021 scanned with T2 mapping pulse sequence were prospectively analyzed.The T2 mapping value of L1-L5 on T2 mapping image was measured through the post-processing workstation.The mean T2 mapping values of lumbar bone marrow and L1-L5 between T2DM group and non-diabetic group were compared.The mean T2 mapping value of lumbar bone marrow in T2DM group stratified by the sex and BMI was compared.The correlation between the mean T2 mapping value of lumbar bone marrow and age in T2DM group and non-diabetic group was analyzed.The mean T2 mapping value of lumbar bone marrow between patients aged≤50 years and>50 years in female and males was compared.Results The mean T2 mapping value of lumbar bone marrow in T2DM group was significantly lower than that of non-diabetic group(P<0.05).The T2 mapping value of L1-L4 in T2DM group was significantly lower than that of non-diabetic group(P<0.05).There was no significant difference in the mean T2 mapping value of lumbar bone marrow of female and male patients,and that between normal weight and overweight patients in T2DM group(P>0.05).The mean T2 mapping value of lumbar bone marrow in T2DM group and non-diabetic group were negatively correlated with age(P<0.05).The mean T2 mapping value of lumbar bone marrow in female aged≤50 years was significantly higher than that aged>50 years(P<0.05).Conclusion The water content of lumbar bone marrow is affected by T2DM,which is correlated with age,but not correlated with BMI.The female hormone level may be an important factor affecting the water content of lumbar bone marrow.
