Characteristics of bone metabolism in elderly male patients with type 2 diabetes

目的:探讨老年男性2型糖尿病(T2DM)患者体内骨代谢指标的变化。方法:选取老年T2DM男性患者229例为研究对象,作为T2DM组,另选取同期健康体检者218例作为对照组,分别测量两组对象的身高、体重,检测两组血清25-(OH)D、血钙、血磷、甲状旁腺激素(PTH)、Ⅰ型原胶原分子N-端前肽(PINP)、β-Ⅰ型胶原C端肽(β-CTX)、低密度脂蛋白(LDL)和高密度脂蛋白检测(HDL)水平。分析、比较各指标之间的关系。结果:与对照组比较,T2DM组的体重指数(BMI)、LDL、β-CTX水平均偏高,差异有统计学意义(P<0.05),而25-(OH)D水平低于对照组,差异有统计学意义(P<0.05);T2DM组血清25-(OH)D水平与血钙呈正相关(相关系数r=0.204,P=0.002);绝大部分研究对象(>80%)存在不同程度的维生素D缺乏或不足。结论:T2DM患者体内骨代谢发生变化,老年患者存在不同程度的维生素D缺乏或不足。

Objective To analyze bone metabolism characteristics in the aged males with type 2 diabetes mellitus(T2DM).Method 229 elderly male patients with T2DM were selected as the study subjects,serving as the T2DM group.218 healthy individuals in the same period were also selected as the control group.The height and weight of the two groups were measured,and serum 25-(OH)D,blood calcium,blood phosphorus,parathyroid hormone(PTH),N-terminal propeptide of type I procollagen fraction(PINP)were measuredβ-C-terminal peptide of type I collagen(β-CTX),low density lipoprotein(LDL)and high-density lipoprotein(HDL)levels.Analyze and compare the relationships between various indicators.Results Compared with the control group,the BMI,LDLβ-The CTX levels were all relatively high(P<0.05);The level of 25-(OH)D was lower than that of the control group(P<0.05);The serum 25-(OH)D level in the T2DM group was positively correlated with blood calcium(correlation coefficient r=0.204,P=0.002);The vast majority of research subjects(>80%)have varying degrees of vitamin D deficiency or insufficiency.Conclusion Bone metabolism has changed in patients diagnosed with T2DM.Vitamin D deficiency or insufficiency is prevalent among aged people.
