老年人群感染新型冠状病毒后肺部炎症的CT表现及动态变化分析

CT findings and dynamic changes of pulmonary inflammation in elderly pati ents infected with novel coronavirus

目的:分析老年人群感染新型冠状病毒(新冠病毒)后肺部炎症的CT表现及动态变化。方法:选择2022年12月至2023年1月广州市东升医院接诊的95例感染新冠病毒并出现肺部炎症的老年患者95例为研究对象,对其胸部CT的表现及动态变化进行分析。结果:95例患者中,出现肺部磨玻璃影、实变影、结节影及小叶内间隔增厚CT表现的患者分别有81例(占85.26%)、79例(占83.16%)、77例(占81.05%)、83例(占87.37%)。95例患者中,出现肺部磨玻璃影、实变影、结节影及小叶内间隔增厚CT表现患者的占比显著高于无此类CT表现的患者,差异有统计学意义(P<0.05)。95例患者胸部CT的动态变化主要为血管充血和血管增粗,其中出现血管充血表现的患者76例(占80.00%),出现血管增粗表现的患者74例(占77.89%)。95例患者中,出现血管充血、血管增粗胸部CT动态变化患者的占比显著高于无此类变化的患者,差异有统计学意义(P<0.05)。结论:老年人群在感染新冠病毒并出现肺部炎症后,其胸部CT会呈现出一些特异性的表现,主要包括肺部磨玻璃影、实变影、结节影以及小叶内间隔增厚等,而随着患者病情的发展,病灶也会呈现出血管充血、血管增粗等方面的动态变化。

Objective:To analyze the CT findings and dynamic changes of pulmonary inflammation in elderly people infected with novel coronavirus(NCOV).Methods:95 elderly patients with COVID-19 infection and pulmonary inflammation were selected from Dongsheng Hospital of Guangzhou from December 2022 to January 2023.The CT findings and dynamic changes of the patients were analyzed.Results:Among the 95 patients,there were 81 cases(85.26%),79 cases(83.16%),77 cases(81.05%)and 83 cases(87.37%)of CT manifestations of pulmonary grinding glass shadow,solid shadow,nodule shadow and intralobular septal thickening,respectively.Among the 95 patients,the proportion of patients with pulmonary grinding glass shadow,solid shadow,nodular shadow and interlobular septal thickening CT findings was significantly higher than tho se without such CT findings,and the difference was statistical ly significant(P<0.05).The dynamic changes of chest CT in 95 patients were mainly vasocongestion and vasothickening,among which 76 patients showed vasocongestion(80.00%)and 74 patients showed vasothickening(77.89%).Among the 95 patients,the proportion of patients with dynamic chest CT changes with vasocongestion and vasothickening was significantly higher than that without such changes,and the difference was statistically significant(P<0.05).Conclusion:After infection with novel coronavirus and pulmonary inflammation in elderly people,chest CT will show some specific manifestations,mainly including pulmonary glass shadow,solid shadow,nodule shadow and thickening of interlobular septum,etc.With the development of patients’disease,lesions will also show dynamic changes in vascular congestion,vascular thickening and other aspects.
