基于数据挖掘的中医药治疗脑卒中后抑郁的用药规律分析

Analysis of Medication Rules of Traditional Chinese Medicine for Post-stroke Depression Based on Data Mining

目的:基于数据挖掘分析中医药治疗脑卒中后抑郁(PSD)的用药规律,为临床治疗PSD提供依据。方法:检索中国知网全文数据库(CNKI)、万方数据库(Wanfang Data)、维普中文科技期刊数据库(VIP)自建库至2021年12月31日收录的中医药治疗PSD的临床文献。纳入的中药组方进行中药频次、功效分类、系统聚类及关联规则分析。结果:共纳入文献175篇,统计中药处方174首,涉及中药167味,使用总频次为1969次,药物使用频次排名前10位的有柴胡、甘草、郁金、川芎、当归、茯苓、白芍、半夏、石菖蒲、香附。使用频次最高的为补虚药,其次是理气药、活血化瘀药。经系统聚类、关联规则分析,聚类树状图获得6个聚类方。关联规则分析得出柴胡-白芍为支持度最高的药物组合,柴胡-龙骨为置信度最高的药物组合。结论:中医药治疗脑卒中后抑郁以疏肝解郁为主,兼以化痰祛瘀、补血活血、理气扶正之法,疏肝不离活血,调肝不忘扶正。

Objective:To analyze the medication rules of traditional Chinese medicine(TCM)in the treatment of post-stroke depression(PSD).Methods:The clinical literatures of the treatment of PSD with traditional Chinese medicine were retrieved from China National Knowledge Infrastructure(CNKI),Wanfang Data and Wipro Chinese Full-text Database of Scientific and Technological Journals(VIP).The frequency,efficacy classification,systematic clustering and association rule analysis were carried out.The retrieving time was from database establishment to December 31,2021.Results:A total of 175 literatures were included,174 TCM prescriptions were counted,involving 167 TCM flavors,and the total frequency of use was 1969 times.The top 10 drugs were bupleurum chinense,glycyrrhiza,Yujin,ligusticum chuanxiong,angelica,poria cohoe,paeony root,pinellia ternata,acorus tatarinoides,and rhizome.The popular medicine were deficiency tonic drugs,followed by qi regulating drugs,blood circulation and stasis removing drugs.The systematic clustering and association rule analysis was performed,the clustering tree could be divided into 6 clustering squares.Association rule analysis showed that"bupleur-paeonia lactiflora"was the drug combination with the highest degree support,and"bupleurus-kegu"was the drug combination with the highest degree confidence.Conclusion:The treatment of post-stroke depression with TCM mainly focuses on regulating the liver to relieve depression.Besides,the methods of resolving phlegm and removing stasis,tonifying blood and promoting blood circulation,regulating the liver and promoting blood circulation are adopted too.
