衰弱评估在慢性心力衰竭患者运动康复中的应用效果

Application effect of frailty assessment in exercise rehabilitation of patients with chronic heart failure

目的探讨衰弱评估在慢性心力衰竭患者运动康复中的应用效果。方法选取2020年1~9月南昌大学第一附属医院收治的慢性心力衰竭患者76例,根据随机数字表法将其分为试验组和对照组,每组各38例。对照组患者实施常规运动康复护理,试验组患者依据衰弱评估结果实施个性化的运动康复计划。比较两组患者干预前后的6min步行试验、左室舒张末期内径(left ventricular end-diastolic diameter,LVEDD)、左室射血分数(left ventricular ejection fraction,LVEF)、N末端脑钠肽前体(N-terminal pro-brain natriuretic peptide,NT-proBNP)、明尼苏达心功能不全生命质量量表(Minnesota living with heart failure questionnaire,MLHFQ)评分、心力衰竭自我管理量表评分。结果干预后,试验组患者的6min步行试验、LVEF及心力衰竭自我管理量表评分均显著高于对照组(P<0.05),LVEDD、NT-proBNP及MLHFQ评分均显著低于对照组(P<0.05)。结论根据衰弱评估结果进行个性化运动康复指导可提高慢性心力衰竭患者的生活质量、活动耐力、自我管理能力,改善心功能,具有临床推广价值。

Objective To explore the effect of frailty assessment in exercise rehabilitation of patients with chronic heart failure.Methods A total of 76 patients with chronic heart failure admitted to the First Affiliated Hospital of Nanchang University from January to September 2020 were selected and divided into experimental group and control group according to random number table method,with 38 cases in each group.The control group received routine exercise rehabilitation nursing,and the experimental group received personalized exercise rehabilitation plan according to the frailty evaluation results.The 6 min walking test,left ventricular end-diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF),N-terminal pro-brain natriuretic peptide(NT-proBNP),Minnesota living with heart failure questionnaire(MLHFQ)score,and heart failure self-management scale score before and after intervention were compared between the two groups.Results After intervention,6 min walking test,LVEF and heart failure self-management scale score in experimental group were significantly higher than those in control group(P<0.05),LVEDD,NT-proBNP and MLHFQ score were significantly lower than those in control group(P<0.05).Conclusion Individualized exercise rehabilitation guidance based on frailty assessment results can improve the quality of life,activity endurance,self-management ability and cardiac function of patients with chronic heart failure,and has clinical value.
