Foley球囊导管在多发性子宫肌瘤核除术后止血中应用研究

The clinical application of Foley catheter in the postoperative hemostasis of multiple uterine myomas

目的探讨Foley球囊导管在多发性子宫肌瘤核除术后止血中应用的安全性及临床疗效。方法选取自2020年2月至2022年12月就诊于辽宁省肿瘤医院及中国医科大学附属盛京医院行手术治疗的多发性子宫肌瘤患者84例,随机将患者分为A组(n=36)与B组(n=48)。两组患者均行子宫肌瘤核除术治疗,其中,B组患者术后应用Foley球囊止血。记录并比较两组患者术后引流情况(第1个24 h引流量、第2个24 h引流量、第3个24 h引流量、术后总引流量及腹腔引流管留置时间)、体温、住院时间及输血情况。结果B组患者术后第1个、第2个、第3个24 h引流量及术后总引流量均低于A组,腹腔引流管留置时间短于A组,差异均有统计学意义(P<0.05)。两组患者术后体温>38.5℃、术后住院时间>5 d患者比例比较,差异无统计学意义(P>0.05)。B组患者术后输血率明显低于A组,差异有统计学意义(P<0.05)。结论Foley球囊导管用于多发性子宫肌瘤核除术后止血安全有效,可减少术后出血,缩短腹腔引流管留置时间,降低术后输血率。

Objective To evaluate the safety and clinical efficacy of Foley catheter in the postoperative hemostasis of multiple uterine myomas.Methods A total of 84 patients with multiple uterine myomas who underwent surgical treatment in Liaoning Cancer Hospital and Shengjing Hospital Affiliated to China Medical University from February 2020 to December 2022 were selected and randomly divided into group A(n=36)and group B(n=48).The patients of the two groups were treated with uterine myomectomy.The patients of groups B were treated with Foley catheter for the postoperative hemostasis.The postoperative drainage conditions(the first 24 hours drainage volume,the second 24 hours drainage volume,the third 24 hours drainage volume,the total postoperative drainage volume and the indwelling time of abdominal drainage tube),body temperature,hospital stay and blood transfusion were recorded and compared between the two groups.Results The drainage volume of the first,second and third 24 hours after operation and the total drainage volume after operation in group B were lower than those in group A,and the indwelling time of abdominal drainage tube was shorter than that in group A,the differences were statistically significant(P<0.05).There was no significant difference in the proportion of patients with postoperative body temperature>38.5℃and postoperative hospital stay>5 days between the two group(s P>0.05).The postoperative blood transfusion rate of group B was significantly lower than that of group A(P<0.05).Conclusion Foley catheter is safe and effective in the treatment of hemostasis after multiple uterine myomectomy.Compared with traditional conservative treatment,Foley catheter can reduce bleeding after operation,shorten duration of indwelling abdominal drainage,and decrease postoperative blood transfusion rate.
