摘要
目的探讨飞行员下腰痛(LBP)患者腰椎MRI影像学的特征性表现。方法采用分层抽样的方法选取2018年1月至2021年1月某空军医院因LBP入院治疗的飞行员75例纳入观察组,选择同期至医院体检的健康飞行员75例纳入对照组。比较两组椎间盘、椎体和终板、小关节及多裂肌的MRI影像学特征。结果观察组中重度椎间盘退变、椎间盘突出、重度小关节退变、小关节积液、Modic改变、Schmorl结节的发生率及多裂肌脂肪浸润率均大于对照组,差异有统计学意义(P<0.05),多裂肌横截面积观察组小于对照组,差异具有统计学意义(P<0.05),椎间盘膨出和纤维环后方高信号的发生率两组之间差异无统计学意义(P>0.05)。结论椎间盘退变、椎间盘突出、Modic改变、Schmorl结节、小关节退变和积液、多裂肌横截面积、脂肪浸润率与飞行员LBP的发生存在相关性,可作为临床诊断飞行员LBP的参考依据。
Objective To investigate the lumbar MRI imaging characteristics in pilots with low back pain(LBP).Methods Stratified sampling was used to select 75 pilots who were hospitalized for LBP in an Air Force hospital from January 2018 to January 2021 and they were included in observation group.A total of 75 healthy pilots who came to the hospital for physical examination during the same period were included in control group.MRI imaging features of intervertebral discs,vertebral bodies,endplates,facet joints and multifidus were compared between the two groups.Results The incidence of severe intervertebral disc degeneration,disc herniation,severe facet joint degeneration,facet joint effusion,Modic changes,Schmorl nodules and fat infiltration rate of multifidus in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).The cross-sectional area of multifidus in the observation group was smaller than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference between the two groups in the incidence of disc bulge and posterior annulus fibrous hyperintensity(P>0.05).Conclusion Intervertebral disc degeneration,disc herniation,Modic changes,Schmorl nodules,facet joint degeneration,facet joint effusion,cross-sectional area and fat infiltration rate of multifidus are related to the occurrence of LBP in pilots,which can be used as a reference for clinical diagnosis of LBP in pilots.
