瑞芬太尼复合丙泊酚在老年患者术中的应用效果

Application effect of remifentanil combined with propofol in elderly patients during operation

目的探讨老年患者在接受腹腔镜下胆囊切除术治疗期间应用瑞芬太尼复合丙泊酚的麻醉效果,为制订老年患者手术麻醉方案提供参考。方法选取2022年1—12月枣庄市峄城区人民医院收治的接受腹腔镜下胆囊切除术治疗的老年患者96例作为研究对象,按照随机数字表法分为对照组和观察组各48例。对照组采取芬太尼复合丙泊酚麻醉,观察组采取瑞芬太尼复合丙泊酚麻醉。比较两组患者血压、心率、恢复自主呼吸时间、拔管时间及苏醒睁眼时间。结果观察组患者插管和拔管时血压水平均低于对照组,心率高于对照组,差异均有统计学意义(P<0.05)。观察组患者恢复自主呼吸时间、拔管时间及苏醒睁眼时间为(5.09±0.55)、(6.11±1.04)、(7.41±1.02)min,均短于对照组的(7.57±0.59)、(10.94±1.07)、(13.34±1.06)min,差异均有统计学意义(P<0.05)。结论老年患者接受腹腔镜下胆囊切除术治疗时采用瑞芬太尼、丙泊酚复合麻醉方案具有良好麻醉效果,有助于减少麻醉对患者心率、血压的影响,且不良反应少,值得临床推广应用。

Objective To explore the anesthesia effect of remifentanil combined with propofol in elderly patients undergoing laparoscopic cholecystectomy,and to provide reference for formulating surgical anesthesia plans for elderly patients.Methods From January to December 2022,96 elderly patients undergoing laparoscopic cholecystectomy admitted to Zaozhuang Yicheng District People's Hospital were selected as the study subjects.They were divided into control group and observation group according to the random number table method,with 48 patients in each group.The control group received fentanyl combined with propofol anesthesia,while the observation group received remifentanil combined with propofol anesthesia.The blood pressure,heart rate,resume spontaneous respiration time,extubation time and wake up and open eyes time were compared between the two groups.Results During intubation and extubation,the levels of blood pressure in the observation group were lower than those in the control group,while heart rate was higher than that in the control group,and the differences were statistically significant(P<0.05).The recovery time of spontaneous respiration,extubation time and wake up and open eyes time in the observation group were(5.09±0.55),(6.11±1.04)and(7.41±1.02)minutes,which were shorter than those in the control group with(7.57±0.59),(10.94±1.07)and(13.34±1.06)minutes,and the differences were statistically significant(P<0.05).Conclusion The combination of remifentanil and propofol anesthesia for elderly patients undergoing laparoscopic cholecystectomy had a good anesthetic effect,which could help to reduce the impact of anesthesia on heart rate and blood pressure of patients,and had fewer adverse reactions,which was worthy of clinical promotion and application.
