不同肿瘤标记物对胃癌转移和预后的预测价值

The predictive value of different tumor markers for metastasis and prognosis of gastric cancer

目的分析不同肿瘤标记物在预测胃癌转移与预后情况方面的临床价值。方法纳入2019年6月—2021年6月期间山东省聊城市高唐县人民医院收治的78例胃癌患者作为研究对象,经电化学发光法测定两项肿瘤标志物,即血清糖类抗原199(CA199)与癌胚抗原(CEA),根据结果将患者分为CA199升高组(n=27例)与CA199正常组(n=51例)、CEA升高组(n=9例)与CEA正常(n=69例)。观察以上组别患者组织病理学参数分布情况(浸润深入、分化程度、淋巴结转移、肝转移、腹膜转移、脏器侵犯、肿瘤直径)以及生存时间。结果CA199升高组组织病理学各项参数与CA199正常组比较差异有统计学意义(P<0.05);CEA升高组组织病理学各项参数与CEA正常组比较差异有统计学意义(P<0.05)。CA199升高组患者生存时间短于CA199正常组;CEA升高组患者生存时间短于CEA正常组,差异均有统计学意义(P<0.05)。结论CA199与CEA可作为反馈胃癌转移与预后价值的两项特异性肿瘤标志物,其升高表示胃癌为晚期事件、生存时间短,且胃癌出现转移。

Objective To analyze the clinical value of different tumor markers in predicting metastasis and prognosis of gastric cancer.Methods A total of 78 patients with gastric cancer treated in Shandong Province Liaocheng City Gaotang County People's Hospital from June 2019 to June 2021 were included as the research objects.Two tumor markers,serum carbohydrate antigen 199(CA199)and carcinoembryonic antigen(CEA),were measured by electrochemiluminescence.According to the results,the patients were divided into CA199 elevated group(n=27 cases)and CA199 normal group(n=51 cases),CEA elevated group(n=9 cases)and CEA normal group(n=69 cases).The distribution of histopathological parameters(depth of invasion,degree of differentiation,lymph node metastasis,liver metastasis,peritoneal metastasis,organ invasion,tumor diameter)and survival time among the groups were observed.Results There were differences in histopathological parameters between CA199 elevated group and CA199 normal group(P<0.05);there were also differences in histopathological parameters between CEA elevated group and CEA normal group(P<0.05).The survival time of patients in CA199 elevated group was shorter than that in CA199 normal group;the survival time of patients in CEA elevated group was shorter than that in CEA normal group;and the differences were statistically significant(P<0.05).Conclusion CA199 and CEA could be used as two specific tumor markers to indicat the value of metastasis and prognosis of gastric cancer.The increasd of CA199 and CEA indicated that gastric cancer was an advanced event,had short survival time and metastasis.
